Antepartum risk factors associated with perinatal outcome in abruptio placentae

Abstract
Objective: To evaluate clinical and laboratory risk factors associated with perinatal outcome in placental abruption. Materials and Methods: Records of 126 patients with abruptio placentae were analyzed retrospectively. Maternal ages, parity, gestational age at birth, maternal complications (vaginal bleeding, placenta previae, hypertension, hypofibrinogenemia, postpartum operatif intervention, blood transfusion requirements), birth weights, stillbirths, Apgar scores,neonatal care unit admissions were reviewed. Multiple logistic regression analyses were used to derive maximum likelihood estimates of the adjusted odds ratios (OR) and 95% confidence intervals were used as measures of the association between pregnancy and outcome and antepartum factors. Results: Multiparity (p=>3) and hypertension was associated with an increase in risk with low Apgar scores (1'<4) (OR=2.8 and OR=2.0 respectively). Hypofibrinogenemia was associated with an increase in risk with stillbirth (OR=5.8), maternal blood trasfusion requirements (OR=5.3) and increased frequency of hysterectomy and/or hipogastric artery ligation (OR=6.6). Conclusion: Gestational age at birth is the most important factor for fetal outcome, where as maternal morbidity is dominated by maternal hypofibrinogenemia.

Amaç: Plasenta dekolmanında fetal sonuçlara ve maternal komplikasyonlara etkili klinik ve laboratuvar faktörleri belirlemek. Gereç ve Yöntem: Plasenta dekolmanı tanısı ile sezaryan olan 126 hasta retrospektif incelendi. Maternal yaş, parite, gebelik haftası, maternal komplikasyonlar (vajinal kanama, placenta previa, hipertansiyon, hipofibrinojenemi, histerektomi ve/veya hipogastrik arter ligasyonu ve kan/kan ürünleri transfüzyon gereksinimi), doğum ağırlığı, ölü doğum, Apgar skorları, yenidoğan servisine çıkış değerlendirildi. Perinatal komplikasyonlara bağımsız olarak etki eden klinik ve laboratuvar parametrelerin saptanması amacı ile çoklu geriaşamalı lojistik regresyon analizi uygulandı. Bulgular: Multiparite (P=>3) ve eşlik eden hipertansiyon düşük Apgar(1'<4)lı bebek doğurma riskini artırmaktadır (sırasıyla OR=2.8, OR= 2.0). Hipofibrinojenemi varlığı ise ölü doğum riskini (OR=5.8), transfüzyon (OR=5.3) ve histerektomi/hipogastrik arter ligasyon (OR=6.6) gereksinimini artırmaktadır. Sonuç: Fetal sonuçlar açısından en önemli faktör gebelik yaşı iken, anne morbiditesini artıran en etkin faktör hipofibrinojenemidir
