Abstract

We aimed to evaluate the factors leading to SIRS and MODS by determining the symptoms, signs and findings of three patients treated in Department of Surgery. Symptoms, signs, findings, characteristics of patients, blood chemistry, count of white cells, red cells and platelets, PaCO2, protrombin time, aPTT, blood, catheter and wound cultures were recorded and evaluated. The ages of the patients were 19, 45 and 56, respectively. All three patients suffered from SIRS and MODS. They also had a lower count of platelets and experienced SIRS equal or longer than 3 days. One patient died of septic shock. MODS may be associated with SIRS from the beginning. Continuing of SIRS at least 3 days and low count of platelets could predict the progression of SIRS to MODS, and low platelet count could predict the presence MODS at the very beginning

Sistemik inflamatuar yanıt sendromu (SIRS) ve multiple organ disfonksiyonu sendromuna (MODS) yol açan belirti ve bulguların cerrahi kliniğinde tedavi edilen 3 hastada saptanarak değerlendirilmesidir. Hastanemiz Cerrahi Kliniği'nde 2004 yılında SIRS tanısı konularak tedavi gören 3 hastanın özellikleri, semptomları, bulguları, kan biyokimyası, beyaz küre, kırmızı küre ve trombosit sayısı, PaCO2, protrombin zamanı, aPTT, kan ve yara kültürleri sonucu kaydedilerek değerlendirildi. Hastalarımızın yaşları sırasıyla 19, 45 ve 56 idi. 3 hastamızda da SIRS'in tüm belirti ve bulguları vardır. Üçünde de trombosit sayısı önemli ölçüde düşmüştür, SIRS 3 gün veya daha fazla sürmüştür ve MODS gelişmiştir, biri septik şoktan eksitus olmuştur. MODS ilk tanı anından beri SIRS'e eşlik edebilir. SIRS'in 3 gün veya daha fazla sürmesi ve trombosit sayısının düşük olması SIRS'in MODS'a ilerleyebileceğinin ve trombosit sayısının düşük olması en baştan MODS'un varlığının habercisi olabilir.
