The correlation among BCL-2, CERB B-2 levels and prognostic factors in the early and locally advanced stage breast cancer

Abstract
AİM: To investigate the correlation among Bcl-2 and C erbB-2 and classical prognostic factors in patients with breast cancer. In addition we aimed to investigate the effects of neoadjuvant chemotherapy to these prognostic factors our patients with stage III breast cancer. MATERİAL AND METHOD: In the study, 20 patients with the diagnosis of fibroadenoma were assigned as the control group (Group 1). There were 4 study groups: 20 patients who were diagnosed as stage I, II breast cancer(Group 2). 20 patients found as stage III breast cancer according to evaluation of the pathological specimens(Group 3), neoadjuvant chemotherapy receiving 16 patients whom diseases were accepted as stage III after physical examination and imaging studies(Group 4), 10 of the group 4 patients who underwent surgical treatment after neoadjuvant chemotherapy (Group 5). Modified radical mastectomy was performed in the study groups and excisional biopsy for the control group. Specimens were evaluated for Bcl-2, C erbB-2. RESULTS: Statistical significance was found for Bcl-2 positivity among the groups(p<0.05). Estrogen and progesteron receptor levels were found significantly higher(p<0.05) in Bcl-2 positive, and lower in C erbB-2 positive patients. In Bcl-2 positive patients axillary lymph node involvement was lesser, and was higher in C erbB-2, but was not statistically significant(p>0.05). Groups 4 and 5 were not statistically different for Bcl-2 and C erbB-2 status(p>0.05). CONCLUSİON: Besides the classical prognostic factors, Bcl-2 and C erbB-2 levels may predict the prognosis and response to the treatment.

AMAÇ: Meme kanserli hastalarda Bcl-2 ve C erbB-2 düzeyleri ile klasik prognostik faktörler olan tümör boyutu, aksiller lenf nodu tutulumu, TNM evresi, grade, östrojen ve progesteron reseptör durumu arasındaki ilişkileri incelemek ve ayrıca evre III vakalarda neoadjuvan kemoterapinin bu faktörleri nasıl etkilediğini araştırmaktır. YÖNTEM VE GEREÇLER: Çalışmada kontrol grubu olarak fibroadenom tanısı almış 20 hasta incelendi (Grup 1). Çalışma grupları olarak, fizik muayene ve görüntüleme yöntemleri ile evre I, II olduğu kabul edilen 20 hasta (Grup 2), cerrahi tedavi sonrası patolojik incelenme ile evre III olduğu saptanan 20 hasta (Grup 3), fizik muayene ve görüntüleme yöntemleri ile önceden evre III kabul edilerek neoadjuvan kemoterapi verilen 16 hasta (Grup 4) ve neoadjuvan kemoterapi verildikten sonra cerrahiye alınan bu 16 hastadan 10'u ele alındı (Grup 5). Çalışma gruplarındaki hastalara MRM, kontrol grubuna ise eksizyonel biyopsi yapılıp, Bcl-2 ve C erbB-2 incelemeleri yapıldı. BULGULAR: Bcl-2 pozitifliği açısından gruplar arasında istatistiksel olarak anlamlı fark görülürken(p 0.05). Bcl-2 pozitif vakalarda östrojen ve progesteron reseptör düzeyleri yüksek iken(p 0.05), C erbB-2 pozitif olgularda ise artmaktadır(p>0.05). Neoadjuvan kemoterapi verilen grupla tümör büyüklüğü açısından grup 5 arasında istatistiksel olarak anlamlı fark görüldü(p 0.05). SONUÇ: Meme kanserlerinde klasik prognostik faktörler dışında Bcl-2 ve CerbB-2 düzeylerine bakılarak prognoz ve tedaviye yanıt hakkında fikir sahibi olunabilir.
