A Review on Doxurubicin Related Genotoxic and Protective Effects of Phytochemicals

Abstract

Doxorubicin is an anthracycline antibiotic that possesses broad spectrum antineoplastic activity and is one of the most widely used chemotherapeutic agents. Doxorubicin is preferred for first-line treatment in both child and adult patients. It is used in various types of cancer such as breast, stomach, thyroid, prostate, testis, over, cervix, soft tissue and bone sarcoma. Like other chemotherapeutic agents, the side effects of doxorubicin are often linked to cytotoxicity against non-selective, normally proliferating cells. In addition, cardiotoxicity and bone marrow suppression limit the use of this drug. Furthermore, during or after chemotherapy treatment, malignant secondary tumors different from primary tumors can develop due to treatment. Free radical production is regarded as the primary mechanism responsible for the genotoxicity of doxorubicin. It causes DNA damage and effects DNA replication resulting in gene mutations, and secondary malignant neoplasms. Phytochemicals play a role in the protection against oxidative damage, the modulation of DNA repair genes and genes controlling cell cycle, the induction of apoptosis, the inhibition of angiogenesis and the regulation of the invasion and spread of tumor cells and epigenetic changes. Many natural compounds with antioxidant properties promise to prevent or reduce the toxic effects of doxorubicin without decreasing its anticancer effect. In this review, the information on the mechanism of action of doxorubicin, toxicity and the protective effects of phytochemicals for reducing this toxicity are given.

Antrasiklin sınıfında yer alan geniş spektrumlu bir antineoplastik ajan olan doksorubisin en yaygın kullanılan kemoterapötik ajanlardan biridir. Hem çocuk hem erişkin hastalarda birinci basamak tedavide tercih edilmekte ve meme, mide, tiroid, prostat, testis, over, serviks, yumuşak doku ve kemik sarkomları gibi çeşitli kanser türlerinde kullanılmaktadır. Doksorubisinin diğer kemoterapötik ajanlar gibi yan etkileri çoğunlukla normal hücrelere karşı selektif olmayan şekilde sitotoksik etki göstermesiyle bağlantılıdır. Bununla birlikte, kardiyotoksisite ve kemik iliği baskılayıcı etkisi bu ilacın kullanımını kısıtlamaktadır. Ayrıca, kemoterapi tedavisi sırasında veya sonrasında, birincil tümörlerden farklı olan kötü huylu ikincil tümörlerin tedaviye bağlı olarak gelişebilmesi diğer önemli bir sorunu oluşturmaktadır. Serbest radikal üretimi, doksorubisinin genotoksik etkisinden sorumlu primer mekanizma olarak kabul edilmekte olup, DNA hasarına neden olması ve DNA replikasyonunu etkilemesiyle mutasyonlara ve ikincil kötü huylu tümörlerin gelişmesine sebep olmaktadır. Fitokimyasallar oksidatif hasara karşı korumada, DNA tamir genlerinin ve hücre siklusunu kontrol eden genlerin modülasyonunda, apoptozun uyarılmasında, anjiogenezin engellenmesinde ve tümör hücresinin invazyonunun ve yayılmasının düzenlenmesinde ve epigenetik değişikliklerde rol oynamaktadır. Antioksidan özelliklere sahip birçok doğal bileşik, doksorubisinin antitümör etkisini azaltmadan, toksik etkilerin önlenmesinde veya azaltılmasında ümit vadetmektedir. Bu çalışmada; doksorubisinin etki mekanizmaları, toksisitesi ve bu toksisiteyi azaltmaya yönelik fitokimyasalların koruyucu etkileri hakkında bilgi verilmesi amaçlanmıştır.
