Bisphosphonates Used in the Treatment of Osteoporosis and Their Toxicities

Abstract

Osteoporosis, is an important health problem that cause morbidity and mortality, is also common in our country. In particular, there is an increase in the incidence of the disease with age, but there are also differences depending on the sex. Various drug groups are used in the treatment of osteoporosis. These drugs are divided into two classes as antiresorptive drugs and anabolic agents. Among these, biphosphonates are drugs that commonly are used and inhibit bone resorption. Biphosphonates have a high affinity for bone mineral. They do not metabolize in the body and interact with other medicines. Oral biphosphonates treatment cause toxicities such as oesophageal ulcers, oesophagitis, and hemorrhages. Renal toxicity, acute phase reactions, jaw osteonecrosis, atrial fibrillation, atypical fractures and ocular adverse reactions have been observed as for result of the long-term use of intravenous biphosphonates. In this review article is summarized definition of osteoporosis, pathophysiology of osteoporosis and osteoporosis treatment, within these the effect mechanisms, physicochemichal proporties, pharmacokinetics, toxicities of biphosphonates and informations about drugs entering this class.

Özet

Osteoporoz, ülkemizde de yaygın olarak görülen, morbidite ve mortaliteye neden olan önemli bir sağlık sorunudur. Özellikle, yaşla birlikte hastalığın insidansında artış gözlenmekle birlikte, cinsiyete bağlı olarak da çeşitli farklılıklar bulunmaktadır. Osteoporoz tedavisinde çeşitli ilaç grupları kullanılmaktadır. Bu ilaçlar, yıkımı önleyici ilaçlar ve anabolik ajanlar olmak üzere iki sınıfa ayrılmaktadır. Bunlar içinde bifosfonatlar, sıklıkla kullanılan ve kemik yıkımını inhibe eden ilaçlardandır. Bifosfonatların kemik mineraline afinitesi yüksektir. Vücutta metabolize olmazlar ve diğer ilaçlarla etkileşime girmezler. Oral bifosfonat tedavisi, özofagus ülserleri, özofajit ve kanamalar gibi toksisitelere neden olmaktadır. Uzun süreli intravenöz bifosfonat kullanımı sonucu ise böbrek toksisitesi, akut faz reaksiyonları, çene osteonekrozu, atriyal fibrilasyon, atipik kırıklar ve oküler advers reaksiyonlar gözlenmiştir. Bu çalışmada; osteoporozun tanımı, patofizyolojisi ve osteoporoz tedavisi ve bunlar içinde bifosfonatların etki mekanizmaları, fizikokimyasal özellikleri, farmakokinetikleri, toksisiteleri ve bu sınıfa giren ilaçlar ile ilgili bilgiler özetlenmiştir.
