The traetment of congenital bilateral lateral patellar luxation seen in calves by transposition of partial patellar tendon and M. uastus lateralis

Abstract
In this study, a method disigned for operative treatment of congenital bilateral patellar luxation (CBLPL) seen in calves and its results were presented. The materials of study were 16 stifle of 8 calves of various breed and sex. The distal 1/4 of femur was exposed out by a skin incision of median para-patellar under the lumbo-sacral spinal anaesthesia. A tunnel was horizantally opened towards femur in the upper point of insertion point of medial femoropatellar ligament to femur by encision of patellar ligament a band (aproximately in 6 cm lengthness and 4 mm wideness) was obtained with isolating of medial 1/3 of patellar ligament from the insertion point of tibia. On the m. vastus lateralis, an second band that is similar size to previous band was obtained from begining of the insertion point of patella towards proximally direction. After application of Locking loop suture on the bands, the free tips of suture materials were passed through from a tunnel in the femur by using a quide dril and pulled out to opposite side. A part of both of tissue band obtained by tightening of suture materials were tracket in to the tunnel of femur, then replaced to patella sulcus trochlea, and the free tips of suture materials were passed through soft around tissues and knotted in the functional position of stifle. The encision side was closed as routinaly. It was observed that all the cases became to normal function in a short time, and no complication were seen on the later periods. It was concluded that the described technique in this study might be succesfully applicable in the stabilization and reduction of patellar luxation in calves.

Bu çalışmada, buzağılarda görülen kongenital bilateral lateral patellar luksasyon (CBLPL)’un operatif sağaltımı için geliştirilen yöntem ve sonuçları sunulmuştur. Çalışma materyalini değişik ırk ve cinsiyette 8 buzağıya ait 16 genu eklemi oluşturdu. Lumbosakral spinal anestezi altında median parapatellar bir deri ensizyonuyla femurun distal 1/4’ü açığa çıkartıldı. Medial femoropatellar ligamentin femura yapışma yerinin üst kısmında femura enlemesine bir tünel açıldı. Patellar ligamentin medial 1/3’ü, tibiaya yapışma noktasından serbestleştirilerek patellaya kadar ensize edilmek suretiyle bir şerit elde edildi (yaklaşık 6 cm uzunluk ve 4 mm genişlikte). M. vastus lateralis’te, patellaya yapışma yerinden başlanarak proksimale doğru uzanan ve patellar ligamenttekine özdeş ikinci bir şerit daha oluşturuldu. Şeritlere locking loop dikişi uyguladıktan sonra ipliklerin serbest uçları bir rehber dril yardımıyla femurdaki tünelden geçirilerek karşı taraftan çıkarıldı. İplikler gerdirilerek elde edilen her iki doku bandının bir kısmının tünelin içine girmesi sağlanarak patella sulcus trochlearis’e yerleştirildi ve ekleme uygun pozisyon verilerek ipliklerin serbest uçları yumuşak dokulardan geçirilerek düğümlendi. Operasyon açıklığı rutin olarak kapatıldı. Tüm olguların kısa sürede normal fonksiyonlarına kavuştuğu gözlendi ve sonraki dönemlerde herhangi bir komplikasyonla karşılaşılmadı. Tanımlanan tekniğin, buzağılarda patellar luksasyonların redüksiyon ve stabilizasyonunda başarıyla uygulanabileceği sonucuna varıldı.
