The accuracy of computerized tomography in classifying the penetrations of the screws inserted in the lumbar vertebrae using the funnel technique, through the pedicles and the corpus as critical or non-critical

Abstract
Objective: Procedures about spinal surgery are generally still performed with conventional techniques using anatomical landmarks due to high costs, long operating processes and difficult techniques of navigation systems used for spinal instrumentation. Although it is checked if there is a perforation inside the canal opened through the pedicule by using probes, penetrations through the corpus and the pedicle inevitably occurs. The main objective of our study is to evaluate the accuracy of computerized tomography in early diagnosis of these penetrations and to take a view on deciding about when it is necessary to revise the application. Method: Pedicle screws were inserted through 200 pedicles of 40 patients who underwent surgery using the Funnel technique and the patients were assessed with computerized tomography scan in early post operative period. Results: In 18 pedicles, the screws were found penetrated. Twelve of them penetrated the lateral cortex of the pedicle and 6 of the screws penetrated the medial cortex of the pedicle. In 2 of the patients the penetrations were determined as critical and the patients were immediately reoperated. During surgery, we observed that the penetrations were critical as revealed by computerized tomography and the screws compressed on nerve roots. No neurovasculary deficit was present in the patients in whom the screw penetrations were determined as non-critical by computerized tomography scan. Twelve of the penetrations which were through the anterior cortex of the corpus and more than 5 mm deep were determined as non-critical. Two of the anterior penetrations deeper than 5 mm were determined as critical Conclusion: In conclusion, computerized tomography scanning is a reliable assessment in the early diagnosis of penetrations and critical /non-critical classification for spinal instrumentation.
Özet
Amaç: Spinal enstrümantasyonda navigasyon sistemlerinin kullanımının halen maliyetli oluşu, işlemin süresini uzatması ve tekniğinin zor olması sebebiyle sıklıkla anatomik yer belirleyiciler kullanılarak spinal cerrahi işlemler gerçekleştirilmektedir. Her ne kadar problar yardımıyla pedikülde açılan kanalda perforasyon olup olmadığı kontrol edilse de pedikül ve korpustan penetrasyonlar kaçınılmaz şekilde gerçekleşmektedir. Çalışmamızın amacı, bu penetrasyonların erken dönemde saptanmasında bilgisayarlı tomografi incelemesinin etkinliğinin saptanması ve saptanan penetrasyonların kritik/non-kritik ayrımının yapılarak hangi durumlarda işlemin revize edilmesi gerektiğine karar verilmesinde bir fikir elde etmektir. Yöntem: Dejeneratif vertebra hastalığı sebebiyle opere edilen 40 hastanın 200 pedikülüne funnel tekniği ile pedikül vidası yerleştirilerek hastaların erken post operatif dönemde lomber bilgisayarlı tomografi taraması yapıldı. Bulgular: Yapılan incelemelerde 18 pedikülde penetrasyon saptandı. Bunların 12’si pedikül lateral korteksinden, 6'sı ise pedikül medial korteksinden penetre idi. 2 hastada penetrasyonların kritik olduğu düşünülerek hastalar acil olarak tekrar opere edildi. Cerrahi esnasında bilgisayarlı tomografide görüldüğü gibi penetrasyonların kritik olduğu ve sinir köküne bası yaptığı görüldü. Non-kritik olarak değerlendirilen penetrasyonların bulunduğu hastaların hiç birinde nörovasküler defisit gelişmedi. Anterior kortekslerinde penetrasyon saptanan 14 vertebra korpusundaki penetrasyonların 5 mm.den az olan 12’si non-kritik, 5 mm.den fazla olan 2’si kritik olarak değerlendirildi. Sonuçlar: Bu sonuçlar göstermektedir ki spinal enstrümantasyonda erken dönemde penetrasyonların saptanması ve kritik/non-kritik ayrımlarının yapılmasında bilgisayarlı tomografi güvenilir bir yöntemdir.
