Selective posterior fusion for lenke type i adolescent idiopathic scoliosis

Abstract

The aim of this retrospective study was to evaluate the results of posterior selective fusion (PSF) performed for patients with Lenke Type 1 Adolescent Idiopathic Scoliosis (AIS). For this purpose 30 patients who underwent PSF for Lenke Type 1 AIS in our institute between 2001 and 2009 have been included in the study. There were 23 females and 7 males. The mean age at the operation was 14.1 ± 1.5 (range 12&#8211;19 years) and the average follow-up period was 46.6 ± 24.2 months (range 15&#8211;108 months). For radiographical assessment, scoliosis parameters including Cobb angles, apical vertebra translations, and coronal and sagittal balance measurement tools were noted using good standing anterior-posterior, lateral, and traction and bending orthoroentgenograms which were taken on preoperative, postoperative and final follow-up periods. Corrected curve magnitudes and correction loss were noted as well. For clinical evaluation SRS&#8211;30 forms were used in the final followup. All data underwent statistical analysis. The mean preoperative thoracic curve Cobb angle which was 48.8º ± 8.09º improved to 16.83º ± 6.85º (correction % 65.51 ± 11.9) (p<0.05) postoperatively and increased to 18.86º ± 1.43º (correction 62.77 ± 12.58 %) (p<0.05) at the final follow-up The mean preoperative lumbar curve Cobb angle which was 29.43º ± 6.58º improved to 10.63º ± 5.71º (correction % 65.04 ± 15.61) (p<0.05) postoperatively and deteriorated as to 12.83º ± 8.41º (correction % 61.79 ± 16.86) (p<0.05) at the final follow-up period. Coronal decompensation in one patient, junctional kyphosis in 2 patients, one superficial wound infection, one implant failure were the complications noted. As a result we conclude that in Lenke Type 1 Adolescent Idiopathic Scoliosis, posterior fusion via selective instrumentation is a safe and effective surgical treatment modality.

Bu çalışmada, kliniğimizde 2001–2009 yılları arasında Lenke Tip 1 Adölesan idiopatik skolyoz (AİS) nedeniyle posterior seçici füzyon (PSF) uyguladığımız hastaların sonuçlarını geriye dönük olarak değerlendirmeyi amaçladık. Çalışmaya dâhil edilen 30 hastadan 23ʼü bayan, 7ʼsi erkek idi. Ameliyat esnasındaki ortalama yaş 14.1 ± 1.5 (dağılım 12–19 yıl) iken, ortalama takip süresi 46.6 ± 24.2 (dağılım 15–108 ay) ay idi. Hastaların radyografik olarak değerlendirmesinde, ameliyat öncesi-sonrası ve son takip döneminde çekilen ayakta önarka, lateral, traksiyon ve eğilme ortoröntgenlerinde skolyoz parametreleri ölçümleri yapıldı. Skolyotik eğriliklerdeki düzelme oranları ve düzelme kayıpları ayrıca kaydedildi. Klinik değerlendirme için son takipte SRS–30 formu kullanıldı. Tüm veriler istatistiksel analize tabi tutuldu. Ana torakal eğrilikte ameliyat öncesi 48.8º ± 8.09º olan ortalama Cobb açısı ameliyat sonrası dönemde 16.83º ± 6.85º (düzelme % 65.51 ± 11.9) (p<0.05) iken son takipte 18.86º ± 1.43º (düzelme % 62.57 ± 12.58) (p<0.05) olarak ölçüldü. Ameliyat öncesi dönemde lomber eğrilikteki ortalama Cobb açısı 29.43º ± 6.58º iken ameliyat sonrası dönemde 10.63º ± 5.71º (düzelme % 65.04 ± 15.61) (p<0.05) olarak ölçüldü ve son takip döneminde 12.83º ± 8.41º (düzelme % 61.79 ± 16.86) (p<0.05) olduğu kaydedildi. Komplikasyon olarak 1 hastada koronal dekompenzasyon, 2 hastada distal kavşak kifozu, 1 hastada yüzeysel yara enfeksiyonu ve bir implant yetmezliği olgusu tespit edildi. Sonuç olarak Lenke tip 1 eğriliklerde posterior enstrümantasyonlu selektif füzyon uygulaması güvenilir ve etkin bir cerrahi tedavi metodu olduğu fikri elde edildi.
