Topical 0.05% cyclosporin A therapy in children with severe vernal keratoconjunctivitis

Abstract

Objective: To investigate the effectiveness of 0.05% cyclosporin A therapy in children with severe vernal keratoconjunctivitis. Material and Methods: Between 2010 and 2012, 19 patients with a diagnosis of vernal keratoconjunctivitis were enrolled in the study. The diagnosis of vernal conjunctivitis was made according to the presence of typical clinical symptoms. The symptoms and signs determined in the patients were separately scored. For each patient, topical cyclosporine treatment was initiated, and at the 3. and 6. months of the therapy, scoring of the symptoms and signs were performed again. Results: The mean age of the participating patients (12 males, 7 females) was 10.1±1.7 years. The scores of the symptoms showed significant improvement during the controls performed at the 3rd and 6th months of the treatment. Despite the scores were not signifi- cantly improved at the 3rd month of the treatment, significant improvements were obser- ved during the controls at the 6th month of the treatment. No adverse effects which caused the cessation of treatment were observed. Conclusion: Topical 0.05% cyclosporine A treatment was proved to cause significant improvement in clinical signs and symptoms in severe patients with active vernal kerato- conjunctivitis. The need for steroids was found to be significantly reduced in these cases. The drug is well tolerated locally.

Özet

Amaç: Ciddi vernal keratokonjonktivitli çocuklarda %0.05 siklosporin A tedavisinin etkinliğini araştırmak. Gereç ve Yöntem: 2010-2012 tarihleri arasında kliniğimizde vernal keratokonjonktivit tanısı alan 19 hasta çalışmaya dâhil edildi. Vernal keratokonjonktivitli olguların tanısı tipik klinik bulgulann varlığına göre konuldu. Tüm hastaların semptom ve bulguları skor- landı. Hastalara topical siklosporin tedavisi başlandı. Üçüncü ve 6. ayda yine semptom ve bulgu skorlamaları yapıldı. Bulgular: Hastaların yaş ortalaması 10.1±1.7 idi. Olguların 12’si erkek, 7’si kadındı. Hastaların semptom skorlarında hem 3. ay hem de 6. ay kontrollerinde belirgin iyileşme saptandı. Hastaların bulgu skorlarında 3. ay kontrollerinde iyileşme belirgin değil iken, 6. ay kontrollerinde belirgin iyileşme ortaya çıkmıştır. Hastaların hiçbirinde tedaviyi kesmeyi gerektiren yan etki ortaya çıkmamıştır. Sonuç: Ciddi aktif vernal keratokonjonktivitli hastalarda topikal %0.05 siklosporin A tedavisi klinik semptom ve bulgularda anlamlı düzelme sağlamıştır. Olgularda steroid gereksinimi belirgin olarak azalmıştır. İlaç lokal olarak iyi tolere edilmektedir.
