Anesthesia management in a child with bilateral vocal cord paralysis

Abstract
In our case report, we discussed our anesthesia procedures in a 3 years old mentally retarded patient who had emergency tracheostomy for sudden-onset near-total pulmonary obstruction. In the first stage of the anesthesia, patient's spontaneous respiration was maintained during diagnostic rigid bronchoscopy the pathology. No pathology other than bilateral vocal cord paralysis was diagnosed by rigid bronchoscopy. In the second stage of anesthesia, orotracheal intubation was carried out followed by uncomplicated emergency tracheostomy. Patient was transferred uneventfully to postoperative intensive care unit after the operation.
Özet
Olgu sunumumuzda, ani gelişen tama yakın solunum tıkanması nedeniyle acil trakeostomi açılan 3 yaşında zeka geriliği olan hastamızdaki anestezi uygulamamızı tartıştık. Anestezi uygulamamızın ilk bölümünde hastanın spontan solunumu korundu ve bu dönemde hastaya tanısal rijit bronkoskopi uygulandı. Rijit bronkoskopi ile bilateral vokal kord paralizisi tanısı konan ve başka bir patoloji saptanmayan hastaya, anestezi uygulamamızın ikinci bölümünde oratrakeal intübasyon uygulandı ve sonrasında sorunsuz olarak acil trakeostomi açıldı. Operasyondan sonra hasta problemsiz olarak postoperatif yoğun bakım ünitesine alındı.
