Decreasing Infection Rates During Long Hospital Stay in a Critical Care Unit

Abstract
Objective: Patients admitted to a critical care unit are frequently exposed to invasive interventions. In some patients the invasive-devices are permanently required to maintain thair lives. As the bedridden patients with need of lifelong care cannot be transferred to a longterm facility care unit they are hospitalized in critical care units for unnecessarily long times. In this study we analyzed invasive-device related infection rates and causative agents in patients hospitalized in a critical care unit over 180 days. Methods: Diagnosis of invasive-device related infections was made according to criteria of “Centers for Disease Control and Prevention (CDC)”. Patient files, hospital automated electronic medical record system, National Hospital Infections Surveillance Network (UHESA) and National Hospital Infections Surveillance Program (INFLINE) were used for the detected infections and their causative agents. Results: The rate of invasive device-related infections decrease consecutively in 90-day-intervals. The specific type of prevalent infection and pertaining causative agents varied at each interval. Conclusion: This study pointed out to the need for long-term care facilities for bedridden patients dependent on invasive devices but not in need for critical care follow-up.

Amaç: Yoğun bakım ünitesine kabul edilen olgular pek çok invaziv girişime maruz kalmaktadırlar. Bazı hastalarda yaşamlarının devamı için invaziv araç gereksinimi kalıcı olmaktadır. Yatağa bağımlı hale gelen ve bakım hastası olan bu olgular başka bir bakım merkezine gönderilemediği için uzun süreler yoğun bakım ünitelerinde yatmaktadırlar. Bu çalışmada, yüz seksen günden daha uzun süre yoğun bakım ünitesinde yatan olgularda her doksan günlük süre içinde saptanan invaziv araç ile ilişkili enfeksiyonlar ve etkenleri incelenmiştir. Yöntem: Enfeksiyon tanıları, “Centers for Disease Control and Prevention (CDC)” kriterlerine göre konulmuştur. Saptanan enfeksiyonlar ve etkenleri için hasta dosyaları, hastane otomasyon sistemi, Ulusal Hastane Enfeksiyonları Sürveyans Ağı (UHESA) ve Ulusal Hastane Enfeksiyonları Sürveyans Programı (INFLINE) kullanılmıştır. Bulgular: Doksan günlük dönemlerde invaziv araç ile ilişkili enfeksiyonların giderek azaldığı saptanmıştır. Her dönemde farklı bir enfeksiyonun öne çıktığı ve enfeksiyona göre etkenlerin de değiştiği görülmüştür. Sonuç: Yoğun bakım ünitesinde invaziv araçlara bağlı ancak yoğun bakım gereksinimi kalmayan yatağa bağımlı bu olgular için uzun dönem bakım merkezlerine olan gereksinime dikkat çekilmek istenmiştir.
