Distal renal tubular acidosis associated with hypercalcemia and nephrocalcinosis in an infant

Abstract

Distal renal tubular acidosis (dRTA) results from ineffective addition of hydrogen ions into the lumen of the distal nephron. The syndrome is manifested by hyperchloremic metabolic acidosis often associated with hypokalemia. Also, hypercalcemia and nephrocalcinosis are usually unexpected in infancy. We describe a 19- day- old full-term boy presented with poor feeding and inactivation. Laboratory investigation revealed hypercalcemia (12.9 mg/dl) and nephrocalcinosis secondary to distal renal tubular acidosis. In conclusion, dRTA should be included in the differential diagnosis of hypercalcemia that may accelerate the development of nephrocalcinosis in the neonate.

Özet

Distal renal tübüler asidoz (dRTA) hidrojen iyonunun distal nefrondan yetersiz sahnımı sonucunda oluşan, sıklıkla hipokaleminin eşlik ettiği, hiperkloremik metabolik asidoz tablosudur. Hiperkalsemi ve nefrokalsinozis ise erken bebeklikte genellikle beklenmeyen bir bulgudur. Bu yazıda, beslenememe ve hareketlerinde azalma yakınması ile gelen ve hiperkalsemi, hiperkalsiüri, hipopotasemi, hiperkloremi ve ağır metabolik asidoza rağmen alkali idrar bulguları ile nefrokalsinozis saptanan 19 günlük dRTA'lu bir erkek olgu sunulmuştur. Bu hasta, hiperkalsemili yenidoğanın ayırıcı tanısında dRTA'u hatırlatmak ve erken tanının önemini vurgulamak amacı ile sunulmuştur.
