Detection of gestational diabetes mellitus prevalance in pregnant patients with oral glucose tolerance test and evaluation of predisposing factors

Abstract
AIM: To determine the prevelance of gestational diabetes mellitus (GDM) and the predisposing factors for GDM in healthy pregnant patients by using oral glucose tolerance test. MATERIAL AND METHOD: The singleton pregnant patients who were admitted to our polyclinic between the dates of April 1st 2004 and April 1st 2005 being smaller than 24 week and not having the diagnosis of either diabetes or gestational diabetes mellitus and not having the history of preeclampsia, hypertension, eclampsia in their previous pregnancies and also not having any systemic diseases were included to our study. During routine follow up, body mass index (BMI) of the pregnant patients between the 24 and 28 weeks was calculated, fasting blood glucose levels were detected following 10-16 hours of starvation, obstetric Doppler US and obstetric anomaly scan. 100 gr oral glucose tolerance test was applied again to those pregant patients following 3 days of 300 gr of carbohydrate containing diet. Than pregnant patients were grouped into patients having GDM or not. Comparisons were ade about terms of age, number of deliveries, family history, weight before pregnancy, body mass index before pregnancy, weight gain up to 24 weeks of gestation. FINDINGS: 6 patients were diagnosed as GDM according to American Diabetes Association. No statistically significant difference was diagnosed in terms of age, number of deliveries, weight before pregnancy and weight at 24 th week of gestation between the groups. In GDM diagnosed group family history of diabetes, body mass index before pregancy and mean weight gain during 24 weeks were statistically significant. CONCLUSION: Gestational diabetes mellitus prevalance was detected as 6% and according to our findings the factor leading to gestational diabetes mellitus were commented as having family diabetes history, higher BMI before pregnancy, higher weight gain during 24 weeks of gestation.

AMAÇ: Bu çalışmamızda sağlıklı gebelerde oral glukoz tolerans testi (OGTT) ile gestasyonel diabetes mellitus (GDM) prevelansını ve GDM için hazırlayıcı olan etkenleri saptamayı amaçladık. GEREÇ VE YÖNTEM: 1 Nisan 2004 ile 1 Nisan 2005 tarihleri arasında polikliniğimize başvuran 24 haftadan küçük gebelikleri olan ve daha önce diyabet ya da GDM tanısı almamış, önceki gebeliklerinde de preeeklampsi, hipertansiyon, eklampsi öyküsü ya da sistemik hastalığı bulunmayan tekil gebeliği olan gebeler çalışmamıza alındı. Gebelerin rutin izlemleri sırasında 24 ile 28. gebelik haftaları arasında vücut kitle indeksleri hesaplandı,10-16 saatlik açlık döneminin ardından açlık kan şekerleri bakıldı, obstetrik Doppler US ve obstetrik anomali US’u yapıldı. Yine bu gebelere 3 günlük 300 gram karbonhidrat içeren diyet sonrası 100 gr oral glukoz ile tolerans testi yapıldı. Daha sonra çalışmaya alınan gebeler OGTT ile gestasyonel diabetes mellitus tanısı konulan ve konulmayan gebeler olarak gruplara ayrıldı ve yaş, doğum sayısı, aile öyküsü, gebelik öncesi ağırlık, gebelik öncesi vücut kitle indeksi, 24. haftaya kadar olan kilo artışı ve 24 haftadaki vücut kitle indeksi açısından karşılaştırıldı. BULGULAR: Amerikan Diabet Kurulu kriterlerine göre 6 gebeye GDM tanısı konuldu. Gruplar arasında yaş, doğum sayısı, gebelik öncesi kilo ve 24. haftadaki kilo açısından istatiksel anlamlı bir fark bulunamadı. GDM saptanan çalışma grubunda ise ailede diyabet öyküsü, gebelik öncesi vücut kitle indeksi ve 24 haftada alınan kilo ortalaması istatiksel anlamlı olacak şekilde yüksek bulundu. SONUÇ: Gestasyonel diabetes mellitus prevalansı çalışmamızda %6 olarak saptanmıştır ve elimizdeki sonuçlara göre gestasyonel diabetes mellitus gelişimine zemin hazırlayan faktörler aile öyküsünde diyabet varlığı, gebelik öncesinde VKİ’nin fazla olması, 24. gebelik haftasında alınan kilonun fazla olması olarak yorumlanmıştır.
