Abstract

Aim: In this study, bacterial colonization was investigated in patients who did not fully respond to standard therapy, whose recovery was delayed despite the absence of significant infection findings. The effect of systemic antibiotic therapy on wound healing was investigated in patients in whom bacterial colonization was detected. Material and Method: A total of 76 patients admitted with venous ulcers between September 2005 and January 2010, and in whom bacterial colonization had been detected were randomly allocated into two groups. While Group I (n:39) received anti- biotic therapy according to the antibiogram in addition to venous insufficiency treatment, Group 2 (n:37) received only ve- nous insufficiency treatment. Both group were compared in terms of wound healing. Findings: The most commonly isolated bacteria was Staphylococcus aureus in both groups. There was no statistically sig- nificant difference between the groups in terms of wound size (p>0.456). However, the time required for wound healing was found to be shorter in the group which received antibiotic compared to the other group. This difference was statistically sig- nificant (p:0.038). Conclusion: A smear should certainly be obtained from patients who do not have significant infection findings, but whose wound healing is delayed. We consider that systemic antibiotic therapy has positive effects on wound healing in patients in whom bacterial colonization is detected.

Özet

Amaç: Bu çalışmada, standart tedaviye yeterli yanıt alınamayan, klinik olarak belirgin infeksiyon bulguları göstermemekle birlikte, iyileşmesi uzamış hastalarda bakteri kolonizasyonu araştırıldı. Bakteri kolonizasyonu saptanan hastalarda sistemik antibiyotik tedavisinin yara iyileşmesi üzerine etkileri değerlendirildi. Gereç ve Yöntem: 2005 Eylül – 2012 Ocak venöz ülser nedeniyle başvuran ve ülserde bakteri kolonizasyonu saptanan 76 hasta rasgele iki gruba ayrıldı. Grup 1 (39) venöz yetmezlik tedavisine ek olarak, antibiyograma uygun şekilde anbiyotik te- davisi aldı. Grup 2 (37) ise, sadece venöz yetmezlik tedavisi aldı. Her iki grup yara iyileşmesi açısından karşılaştırıldı. Bulgular: Her iki grupta da en sık izole edilen bakteri Staphylococcus aureus oldu. İki grup karşılaştırıldığında yara büyük- lükleri açısından istatistiksel olarak anlamlı fark yoktu (p>0,456). Ancak, antibiyotik verilen grupta yara iyileşme süresi, di- ğer gruba göre daha kısa olarak tespit edildi. Bu fark istatistiksel olarak anlamlı bulundu (p: 0,038) Sonuç: Klinik olarak belirgin infeksiyon bulguları olmayan, ancak yara iyileşmesi gecikmiş hastalarda mutlaka sürüntü ör- nekleri alınmalıdır. Bakteri kolonizasyonu saptanan hastalarda sistemik antibiyotik tedavisi yara iyileşmesi üzerine olumlu etki göstermiştir
