Comparison of acute antianginal and antiischemic efficacy of metoprolol, isosorbide mononitrate and nisoldipine in patients with stable angina pectoris and silent ischemia

Abstract
Background and design: In the study, it's aimed to evaluate the acute antianginal and antiischemic effect of oral metoprolol, isosorhide mononitrate and nisoldipine, and effect of these drugs to the exercise parameters in patients with stable angina pectoris and silent ischemia. Methods: The study is performed in between July 1996 - January 1998 prospectively and included 80 patients (18 women and 62 men), all were in 38-71 age group with stable angina pectoris or silent ischemia. All patients performed symptom-limited treadmill exercise. The patients who have positive exercise tests performed again symptom-limited tread mill exercise another day at 2 hours after administration of only one of the drugs: Metoprolol (50 mg), isosorbid mononitrat (50 mg), Nisoldipine (10 mg) or plasebo. Results: The time until development of angina pectoris and 0.1 mm ST segment depression was increased by all three active drugs compared to plasebo; there was no significant difference between active drugs. The time until returning to initial form of ST segment deviation was reduced only by metoprolol, there was no significant difference with other active drugs. The measure of maximal ST segment depression was decreased by all three active drugs. The exercise time was increased by all three active drugs, but only metoprolol prolonged exercise duration compared to plasebo. Conclusion: We concluded that since metoprolol improved ischemic attack more quickly, decreased the measure of maximal ST segment depression more effectively and prolonged exercise duration significantly, it would be preferable to use metoprolol in suitable eases for the treatment of coronary artery disease.

Çalışmada egzersiz testi iskemik açıdan pozitif bulunan stabl angina pektoris ve sessiz iskemisi olan hastalarda oral metoprolol, isosorbid mononitrat ve nisoldipin'in akut antianginal ve antiiskemik etkinliği ile egzersiz sürelerine olan etkilerinin karşılaştırılması amaçlanmıştır. Çalışma Temmuz 1996 - Ocak 1998 tarihleri arasında yaşları 38-71 arasında değişen (yaş ortalaması 57 ± 9) stabl angina pektoris veya sessiz iskemisi olan 18'i kadın, 62'si erkek olmak üzere toplam 80 hastada prospektif olarak yapılmıştır. Olguların hepsine önce semptomla sınırlı egzersiz testi uygulanıp iskemik açıdan pozitif saptananlara başka bir gün Metoprolol (50 mg), İsosorbid mononitrat (50 mg), Nisoldipin (10 mg) veya plasebo olmak üzere dört farklı ilaçtan biri verilerek egzersiz testi ilaç alımından iki saat sonra tekrarlanmıştır. Angina pektorisin ve 0. 1 mm ST segment depresyonunun başlama zamanı metoprolol, isosorbid mononitrat, nisoldipinle plaseboya göre anlamlı olarak uzamış, fakat aktif ilaçlar arasında anlamlı fark bulunmamıştır. ST segment depresyonunun inisiyale dönme zamanı metoprolol grubunda anlamlı olarak azalırken, diğer aktif ilaçlarla anlamlı değişiklik olmamıştır. Maksimal ST segment depresyonu miktarı her üç aktif ilaçla azalmıştır. Egzersiz süresi her üç aktif ilaçla artarken, metoprolol egzersiz süresinde plaseboya göre anlamlı artış yapmıştır. Sonuç olarak metoprololün iskemik atağı daha hızlı düzelttiği, ST segment depresyonu miktarını daha fazla azalttığı ve egzersiz süresinde anlamlı artış yaptığı kanısına varılmıştır.
