A clinical retrospective study concerning acute pseudo-obstruction of colon: Ogilvie syndrome

Abstract
Mechanical intestinal obstruction due to acute pseudo-obstruction of colon was found peroperatively in 5 of the 135 patients who were operated because of ileus in Haseki State Hospital Emergency Clinic in the time period between April-1996 and April-2002. Whether Ogilvie syndrome is a genuine clinical entity or a complication of associated diseases is still on question. One of our patient was treated by tube cecostomy, one of them was treated by transverse loop colostomy, one was treated by total colectomy with ileostomy and the remaining two were treated by decompression via rectal tube peroperatively. All of our patients were discharged from the hospital without any complication.Concerning the two patients who had been treated with rectal tube decompression only, they were rehospitalized because of recurrent mechanical intestinal obstructi-on;one of them for once and the other for twice, respectively. These patients responded to medical treatment. The tube of the patient with tube cecostomy was removed in the fifteenth postoperative day,uneventfully. Because of being in older ages, our patients with colostomy and ileostomy are still living with their stoma burdens and are still under controlI of our outpatient clinic. We presented our patients to remind Ogilvie syndrome, whose diagnosis we could have made only peroperatively in patients with suspicion of mechanical intestinal obstruction. That the endoscopical treatment modalities replaced the preferred surgical approach of the past and the high success rates of medical treatment, both prevent the unnecessary surgical managements nowadays.

Haseki Eğitim ve Araştırma Hastanesi Acil Kliniği'nde Nisan 1996 ile Nisan 2002 tarihleri arasında mekanik barsak obstrüksiyonu nedeniyle ameliyat edilen 135 hastanın 5 tanesinde peroperatif olarak, ileus nedeninin kolonun akut psödoobstrüksiyonuna bağlı olduğu saptandı. Oğilvie sendromu olarak isimlendirilen bu klinik antitenin, halen kendi başına bir hastalık mı, yoksa yandaş hastalıklara bağlı olan bir komplikasyon mu olduğu tam açıklığa kavuşmamıştır. Hastalarımızdan bir tanesine tüp çekostomi,bir tanesine transvers loop kolosto-mi,bir tanesine total kolektomi ve ileostomi ve iki tanesine ise peroperatif rektal tüp ile dekompresyon uygulandı. Hastaların tamamı komplikasyon gelişmeden taburcu edildi. Dekompresyon yapılan hastaların 1 tanesi bir kez, diğeri ise iki kez ileus sebebiyle tekrar interne edildi. Bu hastalar medikal tedaviye cevap verdiler. Çekostomisi olan hastanın tüpü ameliyat sonrası 15.gün çekildi. Kolostomili ve ileostomili hastalarımız ise ileri yaşta olmaları nedeniyle stoma ile yaşamakta olup halen poliklinik kontrolümüz altındadırlar. Mekanik obstrüksiyondan şüphelendiğimiz hastalarda Ogilvie Sendromu olasılığını hatırlatmak amacıyla teşhisini peroperatif koyabildiğimiz hastalarımızı sunduk. Geçmişte tedavide tercih edilen cerrahi yaklaşımın günümüzde yerini endoskopik tedavi modalitelerine terk etmiş olması ve medikal tedavinin başarısı, gereksiz cerrahi girişimi engellemektedir.
