The comparison of computed tomography and ultrasonography in patients with blunt abdominal trauma

Abstract
Purpose: The aim of this study was to compare the US and CT findings in blunt abdominal trauma. Materials and methods: Patients who were exposed to blunt abdominal trauma were evaluated by US and CT. Results: In the 25 patients we examined, we performed US in 24, parenchymal nonhomogeneity was inspected in 2, but the findings were not as discrete as it were with CT. We couldn't perform US on one of the patients who was in a non-steady state. Organ injury was shown in all of the 24 patients on which CT was performed. In a patient that was pregnant, CT was not performed. In 2 of the patients multisystem trauma was shown with CT, while it could not be inspected by US. The findings were confirmed with-surgery in 4 patients; and with clinical and radiological survey in the others. As a conclusion no significant difference was found statistically between the findings of US and CT in blunt abdominal trauma. (X2=0.18, P=0.98, P>0.05) Discussion: With appropriate indication and with appropriate preference both modalities can be used in patients with blunt abdominal trauma.

Amaç: Bu çalışmada, künt abdominal travmalarda US ve BT bulgularının karşılaştırılması amaçlanmıştır. Araç ve Yöntem: Künt abdominal travmaya maruz kalan hastaların US ve BT incelemeleri yapılmıştır. Bulgular: 25 hasta üzerinde yaptığımız çalışmada 24 hastaya US yapılmış ve 22'sinde organ yaralanması gösterilirken 2 hastada parankimal nonhomojenite görülmüş ancak BT kadar net tanımlanamamıştır. 1 hastaya genel durum bozukluğu nedeniyle US incelemesi yapılamamıştır. 24 hastaya BT uygulanmış olup hepsinde organ yaralanması saptanmıştır. Gebelik nedeniyle 1 hastaya BT çekilememiştir. 2 hastada, US ile gösterilmesi mümkün olmayan multisistem yaralanmalar BT ile gösterilmiştir. Bulgular 4 hastada cerrahi uygulanarak, diğerlerinde klinik ve radyolojik takip ile doğrulanmıştır. Sonuç olarak US ve BT bulguları arasında farklılık istatistik yönden anlamlı bulunmamıştır. (X2=0.18, P=0.98, P>0.05) Tartışma: Künt abdominal travmalı hastalardaki organ yaralanmalarının değerlendirilmesinde uygun endikasyon ve doğru seçim ile her iki yöntemdende faydalanılabilir.
