Evaluation of clinical and laboratory features of 22 patients with brucellosis

Abstract
22 patients with brucellosis have been investigated for clinical and laboratory features and complications of the disease. 11 of the cases were acute, 9 were subacute and 2 were diagnosed to have chronic brucellosis. 18 (82%) of the patients had fever, 15 (68%) had sweating, 15 (68%) had arthralgia and 11 (50%) had fatigue. The most frequent findings were fever in 15 (68%), hepatomegaly in 9 (41%), splenomegaly in 5 (23%), osteoarticular involvement in 9 (41%), central nervous system involvement in 2 (9%), cardiovasular system involvement in 1 (5%) patient. In 16 patients elevated erythrocyte sedimentation rate, in 16 patients anemia, in 10 patients lymphocytosis, in 1 patient lymphopenia, in 6 patients monocytosis, in 1 patient leucopenia and in 1 patient leucocytosis were observed. In 5 (23%) patient Brucella spp. was isolated from the blood cultures. In 21 patients standard Brucella tube aglutination test was positive (≥1/160 dilution). The therapy of patients were planned in accordance with organ involvement and response to therapy in all our patients was achieved. There was no unresponsiveness to therapy or relapse in our cases.
Özet
Bruselloz tanısı konulan 22 hasta klinik, laboratuvar özellikleri ve gelişen komplikasyonlar açısından değerlendirildi. Olguların 11'i akut, 9'u subakut, 2'si kronik bruselloz idi. Hastaların 18 (%82)'inde ateş, 15 (%68)'inde terleme, 15 (%68)'inde eklem ağrısı, 11 (%50)'inde halsizlik yakınması mevcuttu. En sık saptanan bulgular ateş 15 (%68), hepatomegali 9 (%41), splenomegali 5 (%23), osteoartiküler tutulum 9 (%41), santral sinir sistemi tutulumu 2 (%9) ve kardiyovasküler sistem tutulumu 1 (%5) idi. 16 hastada eritrosit sedimentasyon hızında artış, 16 hastada anemi, 10 hastada lenfositoz, 6 hastada monositoz, 1 hastada lenfopeni, 1 hastada lökositoz ve bir hastada lökopeni saptandı. 5 (%23) hastada kan kültüründe Brucella spp. üredi. 21 hastada standart tüp aglütinasyon testi ≥1/160 dilüsyonun üzerinde pozitif olarak saptandı. Hastaların tedavileri organ tutulumlarına göre planlandı ve tüm tedavi rejimlerine yanıt alındı. Olgularda verilen tedaviye yanıtsızlık veya relaps gözlenmedi.
