Comparison of total intravenous anesthesia (TIVA) and pentothal-halothane anesthesia in pediatric tonsillectomy operations

Abstract
As we know, tonsillectomy due to tonsillary hypertrophy is commonly performed in pediatric age group. In this group of patients, a fast and nonproblematic postoperative recovery is mandatory, because these patients are regarded as outpatients. In this study, we to¬ok 50 pediatric patients categorized as ASA-1 group and divided them into two random groups. The first group received 5-7 mglkg pentothal for induction and the second group received 2jiglkg alfentanyl and 2,5mglkg propofol. Both groups received 0,5mglkg atracu-riumfor the maintenance halothane was applied to the first group, 9mglkglh propofol infusion was provided for the second group. Both groups received 59 % N20 and 02. We noticed, early and fast recovery in the second group. We concluded that TIVA (total intravenous anesthesia) is the choice of method of anesthesia which should be applied in operations requiring early and fast recovery like tonsillectomy.
Özet
Bilindiği gibi pediatrik yaş grubunda, tonsil hipertrofisi nedeniyle tonsillektomi operasyonu çok sık yapılmaktadır. Bu grup hastalarda özellikle postoperatif derlenmenin hızlı ve problemsiz olması arzu edilmektedir. Çünkü bu hastalar outpatient grubu hastalardır. Çalışmamıza, ASA-I grubundan 50 çocuk hasta alındı. Hastalar rastgele iki gruba ayrıldı. Birinci gruba, indüksiyonda pentothal 5-7mg/kg, ikinci gruba alfentanyl 2ug/kg ile propofol 2,5mg/kg uygulandı. Her iki gruba Atrakuryum 0,5mg/kg verildi. İdame için birinci gruba Halotan %1-1,5, ikinci gruba propofol 9mg/kg/h infüzyonu sağlandı. Her iki gruba N2O ve O2 % 50 oranında verildi. Çalışmamızda, ikinci grupta derlenme daha çabuk ve daha rahat gerçekleşti. Bu nedenle tonsillektomi gibi erken derlenmenin önemli olduğu girişimlerde, TİVA(total intravenöz anestezi )'nin iyi bir anestezi yöntemi olduğuna karar verildi.
