Comparison of monotherapy and combined androgen blockade in advanced prostate cancer

Abstract
In Europe, prostatic carcinoma (CaP) has become the second most common form of cancer in males, and the disease shows increasing incidence rates. In the United States, lifetime risk of having a CaP was at advanced stage (C, Dİ, D2) in 29,8% of case of first presentation. In CaP treatment, aim of hormonal therapy, remove androgens or block their celluler effects. With this purpose, surgical and medical castration is performed and serum androgen levels must be kept at low levels. With these treatment modalities. It has been suggested that androgens which is derived from adrenal gland also inhibit stimulation of prostate and therefore time of without progression will be long. The aim of this study was to compare the effects of the nonsteroidal antiandrogen flutamide plus the LH-RH analogue triptorelin acetate (combined androgen blockade, [CAB]) with triptorelin acetate alone in patients with advanced prostate cancer.
Özet
Avrupada; prostat kanseri, erkekler arasında görülen en yaygın ikinci kanser formudur, özellikle hasta sayısında artış gözlenmektedir. ABD'de, prostat kanserine sahip siyahlarda hayatta kalım riski %9,4 iken, beyazlarda %8,7'dir. Prospektif bir çalışmada, prostat kanserine sahip olguların yaklaşık %29,8'inde ilk başvuruları sırasında ileri evre olduğu (C, Dİ, D2) bildirilmiştir. Metastatik prostat kanseri tedavisinde hormonal tedavinin amacı, hücresel düzeyde androjenlerin etkilerini baskılamaktır. Bu amaçla, cerrahi ya da medikal kastrasyon yapılmakta ve serum androjen düzeyleri düşük seviyede tutulmaya çalışılmaktadır. Bu tedavi şekilleri ile sürrenal kaynaklı androjenlerin baskılanmasının progresyonsuz süreyi uzatacağı sanılmaktadır. Bu çalışmanın amacı, metastatik prostat kanserinde nonsteroidal antiandrojen olan flutamide arttı bir LH-RH analoğu olan triptorelin asetat ile sadece triptorelin asetatın karşılaştırılmasıdır.
