Current management of the brain pyogenic absceses treatment experience with 50 cases

Abstract

This retrospective study includes 50 patients with brain abscess diagnosed in CT. The goal of this study is to better define the roles of surgery (open or aspiration) and medical management in patients with brain abscesses. Between 1990-1998,50 patients with brain abscess were treated by either only open surgery or aspiration technique. The ages of the patients ranged from 2 months to 70 years (median,19.9 yr). Most frequent symptom was headache (% 74.4) In all patients, the diagnosis of brain abscess was made by computed tomography (in all patients) magnetic resonance imaging was made in 31 patients (62.%). 15 patients had multipl abscesses.12 patients had brain abscesses located in the posteior fossa area. 33 surgical procedures were performed in 26 patients. Mortality rate is open surgery was 6 %. In 24 cases gone under aspiration tecnique mortality rate was 3.12% and morbidity rate was 6.24%. In clinically stable patients with deeply located abscesses and in cases with small abscesses in cerebritis or capsular stage only initial medical therapy may be started. Biweekly CT scans must be done to monitor the response to treatment. CT quided aspiration should be performed in the event of clinical deterioration, failure of reduction in size or enlargement of abscesses. Exicision is required in large superficial absceses resistant to multiple aspirations, post-traumatic abscess with a foreign body or fistula and multiloculated abscess of nocardial or actinomycotic aetiology. Aspiration or open surgery combined with an 8-week intravenous antibiotic regimen has yielded an effective therapeutic result in all of our abscesses, small or large, solitary or multiple, superficial or deep seated. We do not recommend open surgical procedure as the first step. Aspiration of the abscess followed by antibiotic therapy should yield cure rates of more than 85 % in patients with multiple or solitary brain abscesses.

Özet

Bu retrospektif çalışma beyin tomografisi ile tanısı konmuş 50 olguyu kapsamaktadır. Çalışmanın ana amacı beyin apselerinde açık cerrahi, aspirasyon ve medikal tedavinin etkinliğini araştırmaktır. 1990-1998 yılları arasında müdahele görmüş (cerrahi, aspirasyon veya her ikisi ). Hastaların yaşı 2 ay ile 70 yaş arasında olup ortalama yaş 19.9 bulunmuştur.En sık semptom baş ağrısıdır (% 74.40), Tüm hastalara beyin tomografisi ile tanı konurken ilaveten 31(%62) hastaya MRG yaptmlabilmiştir.12 olguda (% 24) apse arka çukur lokalizasyonun da idi. 26 olguya toplam 33 cerrahi müdahale yapılmıştır (nüks, rezidu vd). Açık cerrahi ile mortalite oranı %6 ve aspirasyon tekniği uygulanan 24 olgu da mortalite oranı %3.12 iken, morbidite %6.24 bulunmuştur. Klinik olarak stabil olan ve derinde yerleşmiş ve serebritis fazındaki hastalara sadece antibiyotik tedavisinin başlanması uygun olacaktır. İki hafta sonra çekilecek beyin tomografisinden sonra apsenin çapında azalma olmaz ise veya büyüme tespit edilirse aspirasyon tekniği denenmelidir. Açık cerrahiye alınması gereken olgular ise nocardiya ve akçinomiçes apseleri, postravmatik yabancı madde içeren apseler ve aspire edilmesine rağmen küçülmeyen, yüzeyel yerleşimli çoğul olan apselerdir. Bu işlemlerle birlikte tedavinin effektif olabilmesi için hastaların 8 hafta antibiyotik tedavisi almaları gerekir. Sonuç olarak aspirasyon tekniği, lezyon gerek soliter gerek multipl olsun hastaların %85 iyileşmelerine neden olmaktadır. Bu nedenle öncelikle açık cerrahi önermemekteyiz.
