The effects of valsartan to microalbuminuria and to the levels of serum bun and creatinine in hypertensive type II diabetic and normotensive type II diabetic patients

Abstract
Diabetic nepropathy is frequent and is an important microvascular complication. The hypertensive diabetic patients are more likely to have microvascular and macrovascular complications than normotensive diabetics. Essential hypertension also causes nephropathy as a target organ damage and the earliest laboratory diagnosis of it is microalbuminurea. In this study the patients were chosen among the patients of the hypertension and internal diseases policlinics. In total 44 patients were included. 30 of them were hypertensive Type II diabetic and 14 of them were normotensive Type II diabetic patients. At the begining of the study initial blood and urine examples were taken and 80 mg I day Valsartan therapy was started. The study lasted for 16 weeks and the biochemical measurements and microalbuminuria levels were repeated. In this study we examined the effects of valsartan (a selective Angiotensin II receptor antagonist) to the microalbuminuria, serum BUN and creatinin levels in Type II diabetic patients. We also examined the usefulness, tolerability and side effects of valsartan therapy. In the normotensive group there was no significant effect of valsartan on microalbuminuria, but in the hypertensive type ll diabetic group the levels of microalbuminuria was significantly decreased. When the serum BUN and creatinin levels before and after the valsartan therapy were compared there was no statistically meaningful difference. The drug was well tolerated in both groups Our study supports that AT II receptor blockers are effective at preventing the development and progression of diabetic nephropathy and they are well tolerated.
Özet
Diabetik nefropati sık rastladığımız önemli bir mikrovasküler komplikasyondur. Diabetiklerde mikrovasküler ve makrovasküler komplikasyonlar hipertansiyonun eklenmesiyle artar. Diabetes mellitus gibi esansiyel hipertansiyon da hedef organ hasarı olarak nefropatiye yol açar ve bunun da en erken laboratuar bulgusu yine mikroalbuminüridir. Bu çalışmaya alınan hastalar Dahiliye ve Hipertansiyon polikliniğine başvuran hastalar arasından seçildi. 30'u hipertansif Tip II diabetik, 14'ü ise normotansif Tip II diabetik olmak üzere toplam 44 hasta çalışmaya alındı. Çalışmanın başlangıcında kan örnekleri ve 24 saatlik idrar örneği alınarak 80 mg/ gün tek doz Valsartan başlandı. 16 haftalık Valsartan tedavisi sonrasında hastaların biokimya ve 24 saatlik idrarda mikroalbüminüri tayini tetkikleri tekrar edildi. Çalışmamızda bir Anjiotensin II reseptör antagonisti olan Valsartan'ın bir uç organ hasarı olarak diabetik nefropatinin erken göstergesi olan mikroalbüminüriye etkisini, serum BUN ve kreatinin değerlerine etkisini ve etkinliği ile tolerabilitesini araştırdık. Normotansif diabetik hasta grubunda mikroalbüminüriye anlamlı etkisi olmadığını, hipertansif diabetik hasta grubunda ise mikroalbüminüriyi azalttığını ve iyi tolere edildiğini gördük. Her iki grupta da tedavi öncesi ve sonrası serum BUN ve kreatinin düzeyleri karşılaştırıldığında anlamlı fark bulunmadı. Çalışmamız AII reseptör blokerlerinin diabetik nefropatinin gelişmesini önleme konusunda etkili olduğunu ve iyi tolere edildiğini desteklemektedir.
