A comparative study about the effect of bupivacaine against the combination of bupivacaine with phentanyl on the analgesy time in sectio operations with spinal anaesthesia

Abstract
Recently the neuroaxial use of opioids became more popular because of their synergic effects with local anaesthetic drugs. In this study we aimed to evaluate the effect of phentanyl add to local anaesthetic drugs on the period of analgesia. We included 50 ASA 1 elective sectio cases aged between 19-35 years. These were divided in two groups. Each containing 25 cases. The first group received spinal anaesthesia with 10 mg of %0.5 isobaric bupivacaine where as in the other group spinal anaesthesia performed with the combination of 10 mg of bupivacaine and 15 mcg phentanyl for the lumber punction we used the space between L3 and L4. In the second group the analgesic period was significantly longer than the period in first group. As the result of this study the phentanyl add (we used 15 mcg) to local anaesthetic provided us prolonged postoperative analgesia and only in a few cases (5 cases) itching was noticed as an adverse effect. Our opinion is that the additional opioid in the doses we used is suitable for an uncomplicated and effective postoperative analgesic.
Özet
Çalışmamızda lokal anestezik ilaçlara fentanil ilavesinin analzeji süresine etkisini araştırmak amacı ile 19 - 35 yaş arası ASA 1 grubu 50 elektif sectio vakası 25'er kişilik iki gruba ayrıldı. 1. gruba %0.5 izobarik bupivacaine 10 mg ile spinal anestezi uygulandı. 2. gruba %0.5 bupivacaine 10 mg + 15mcg fentanil ile spinal anestezi uygulandı. 2. Grupta analjezi süresi 1. Gruba nazaran daha uzun sürdü ve istatistiksel olarak anlamlı bulundu. Sonuç olarak çalışmamızda kullandığımız fentanil dozu (15 mcg) uzamış postoperatuar analzeji sağladı ve çok az vakada (5 vaka) yan etki olarak kaşıntı gözlendi. Bu dozda ilave opioidin etkin ve sorunsuz bir post operatuar analjezi için uygun olduğunu düşünmekteyiz.
