Current status of open surgical treatment protocols for subacromial ımpingement syndrome associated with rotator tear

Abstract

Objective: The aim of this study was to evaluate the recent treatment protocols for Neer stage III subacromial impingement syndrome with open anterior acromioplasty and rotator cuff repair. Material and Methods: Twenty-two patients (8 males, 14 females; mean age: 52.9±10.2) who were diagnosed with a rotator cuff tear based on clinical and radiological findings between 2009 and 2010 participated in the study.. We used the open surgical decompression technique which was previously described by Neer. The ruptured tendon ends were isolated and were fixed to the bones with appropriate suture anchors and transosseoz sutures. Preoperative, postoperative and the final follow-up Constant and UCLA shoulder scores were evaluated. Results: The mean preoperative Constant score was 34.4±6.6 and UCLA score was 13.8±3.3. The mean postoperative Constant score was 73±7.6 and UCLA score was 31.7±3.3. Significant improvement was observed in postoperative shoulder scores (p<0.01) and postoperative shoulder range of motion in all patients (p<0.01). Conclusion: Currently, clinical and functional results of open and arthroscopic subacromial rotator cuff decompression are similar. However, many surgeons prefer the open method and achieve successful results. (The Medical Bulletin of Haseki 2012; 50: 59-63)

Özet

Amaç: Bu çalışmadaki amacımız Neer’e göre evre III subakromiyal sıkışma sendromu tedavisinde anterior açık akromiyoplasti ve rotator manşet onarımının güncel tedavi yaklaşımlarında yerini tartışmaktı. Yöntem ve Gereç: Kliniğimizde 2009-2010 yılları arasında klinik ve radyolojik olarak rotator manşet yırtığı tanısı konmuş 22 hasta çalışmaya alındı. Olguların sekizi erkek (%36.4), on dördü kadın (%63.6), ortalama yaş 52.9±10.2, dağılım 37-72 idi. Cerrahi uygulamalar daha önce Neer tarafından tarif edilen açık subakromiyal dekompresyon tekniğe uygun olarak yapıldı. Rotator manşette izole edilen yırtık tendon uçları hastaların kemik kalitesine göre belirlenen ankor sütür tipiyle veya transosseöz sütürlerle kemiğe tespit edildi. Olguların hepsi ameliyat öncesi ve ameliyattan sonraki son kontrollerinde Constant ve UCLA (University of California at Los Angeles) skorlama sistemleri ile değerlendirildi. Bulgular: Olgularımızın ameliyat öncesi Constant skoru ortalama 34.4±6.6, UCLA skoru ortalama 13.8±3.3 olarak bulundu. Ameliyat sonrası değerlendirmede ise ortalama Consant skoru 73±7.6, UCLA skoru 31.7±3.3 olarak tespit edildi. Ameliyat öncesi ve sonrası bu skorlardaki artış ileri düzeyde anlamlı olarak tespit edildi (p<0.01). Tüm olgularda ameliyat sonrası eklem hareket açıklıklarındaki artış ileri düzeyde anlamlı bulundu (p<0.01). Sonuç: Günümüzde açık ve artroskopik yöntemlerle yapılan subakromiyal dekompresyon ve rotator manşet tamirlerinin klinik ve fonksiyonel sonuçları benzerdir. Ayrıca halen çoğu cerrah açık yöntemleri tercih etmekte ve başarılı sonuçlar elde etmektedir.
