Comparison of Tramadol, Diclofenac and Fentanyl in Combination with Propofol for Dilatation and Curettage: A Retrpospective Study

Abstract
Aim: The aim of our study was to compare the effects of preoperative use of tramadol, diclofenac and fentanyl in combination with propofol on hemodynamics, propofol requirements, recovery and post-operative pain in patients undergoing dilatation and curettages. Methods: After approval of the Ethics Committee, 90 patients aged 18-70 years, who had undergone a dilatation and curettage between April 2009 and March 2011, were included in the study. The subjects were divided into three groups: group D consited of patients who received diclofenac 100 mg intramuscularly 30 minutes before induction, group T received tramadol 100 mg intravenously 15 minutes before induction and the patients in group F were administered fentanyl 1 µgr/kg intravenously for induction. Induction was provided with propofol 2 mg/kg and the total doses of propofol required were recorded. Preoperative and postoperative hemodynamic parameters and SpO readings were recorded. The patients were evaluated using a visual analog scale , the Ramsay Sedation Scale , and the Aldrete Recovery Scale at the 1st, 5th, 10th, 120th minutes postoperatively. Results: The mean postoperative systolic blood pressure and the mean arterial blood pressure were significantly lower in group F. In the first minute, Aldrete recovery scores were significantly lower in G, group D and group T. Propofol consumption was significantly lower in group F. Conclusion: Fentanyl decreased the propofol requirements and increased the recovery score accordingly. Hypotension should be considered in high-risk patients. There was no statistically significant difference between the groups regarding effectiveness of analgesia. It is concluded that each can be used alternatively prior to dilatation and curettage

Amaç: Çalışmamızda dilatasyon ve küretaj girişimlerinde preoperatif tramadol, diklofenak ve fentanilin kullanımının hemodinami, propofol dozları, derlenme ve postoperatif ağrı üzerine etkilerinin karşılaştırılması amaçlanmıştır. Yöntemler: Etik kurul onayı alındıktan sonra, geri dönük olarak planlanan çalışmaya 18-70 yaş arası 90 hastanın verileri dahil edildi. Hastalar 3 grup olarak incelendi. Grup D'de indüksiyondan 30 dk önce intramusküler diklofenak 100 mg, Grup T'de indüksiyondan 15 dk önce intravenöz tramadol 100 mg, Grup F'de indüksiyonda intravenöz fentanil 1 µgr/kg dozunda uygulandı. Hastalara indüksiyonda 2 mg/kg propofol uygulandı, ek doz ihtiyaçları Ramsey sedasyon skalası ile değerlendirilerek Ramsey 3-4 arasında tutulmaya çalışıldı. Uygulanan toplam propofol dozları kaydedildi. Maske ile oksijen altında operasyona alınan hastaların peroperatif ve postoperatif Sistolik Kan Basıncı (SKB), Diyastolik Kan Basıncı (DKB), Ortalama Kan Basıncı (OKB), Kalp Atım Hızı (KAH), Oksijen Satürasyonu (SpO) kaydedildi. Postoperatif 1, 5, 10, 120. dakikalarda Visual Analog Skala (VAS) (0-10 arasında), Ramsey Sedasyon Skalası (0-6 puan arası), Aldrete Derlenme Skalası (0-10 puan arası) değerlendirildi. Bulgular: Grup F'de postoperatif SKB ve OKB değerleri anlamlı derecede düşük bulundu. Grup D ve Grup T'de 1. dakika Aldrete derlenme skorları anlamlı derecede düşük bulundu. Grup F'de toplam propofol tüketiminin anlamlı derecede düşük olduğu gözlendi.Sonuç: Fentanilin kullanılan propofol dozunu azalttığı bunun neticesinde derlenme süresini kısalttığı sonucuna varıldı. Analjezik etkinlik açısından ise tramadol, diklofenak ve fentanilin dilatasyon ve küretaj girişimlerinde birbirlerine üstünlüklerinin olmadığını gözlemledik
