Abstract

OBJECTIVE : The present study was undertaken to verify the efficacy of different ovulation induction methods for inducing follicular maturation and ovulation in patients with Polycystic Ovary Syndrome (PCOS). , STUDY DESIGN : Clomiphene citrate ( CC ) was given to 132 patients with PCOS asked us for infertility due to anovulation. Among the unconceived patients with CC, 32 of them had undergone ovarian electrocautery and ovulation induction with as 8 HMG, 12 GnRH-a + HMG and 45 pFSH was carried out for total 65 patients. We evaluated these ovulation induction protocols according to efficacy, reliability and results. RESULTS : 132 patients were treated in 348 cycles with 50-100-150 mg/day of CC with pregnancy rates 54%, 29% and 1 i% respectively. Ovarian electrocautery was successfully used with 78% ovulation and 34% pregnancy rates. Ovulation induction with HMG after hypophyseal suppression had no better results than HMG and pFSH alone. There was no meaningful difference or superiority of each other between pFSH and HMG according to pregnancy, ovulation and hyperstimulation rates. CONCLUSION : CC must be the first line of the anovulation therapy in PCOS patients. Ovarian electrocautery combined with other parenteral induction agents, GnRH-a + HMG, HMG and pFSH or can not get good results for anovulation therapy in PCOS. There is no difference between pFSH and HMG according to different variables. Addition of GnRH-a to gonadotropin induction cycles does not mean a decrease in multifollicular development.
