Abstract

OBJECTIVE: We analysed the prognostic factors, clinical variables, recurrences, disease free intervals, treatment modalities and outcomes in clinical stage I and II endometrial cancer with positive peritoneal cytology. STUDY DESIGN: 94 patients with endometrial cancer seen from August 1990 to October 1995 were evaluated according to prognostic factors and whether peritoneal washings did or did not contain malignant cells. Clinical stage I and II patients with positive cytology were in turn analysed with respect to their accompanying prognostic and clinical variables recurrences, disease free intervals, treatment modalities and final outcome. RESULTS: Clinical early stage endometrial cancer with-malignant peritoneal cytology has similar distributions of tumor grades, extent of myometrial invasion, lymph node involvement, histologic subtypes and age as compared to those in patients with normal washings. There is a trend for increased risk of recurrence with positive cytology. CONCLUSIONS: Positive cytology acts as an independent prognostic factor in clinical early stage endometrial cancer and the poor prognosis associated with malignant washings is not merely a reflection of other adverse prognostic factors. The value of adjuvant therapy in this setting needs further clarification.

Ağustos 1990 ve Ekim 1995 tarihleri arasında A.Ü.T.F Kadın Hastalıkları ve Doğum A.B.Dda tedavi edilen 42 klinik evre I ve II endometrium kanserli hasta, eşlik eden prognostik ve klinik değişkenler, rekürrensler, hastalıksız dönem, tedavi modaliteleri ve yaşam süreleri açısından analiz edildi. Malign peritoneal sitolojiye sahip klinik erken evre endometrium kanserleri normal sitolojiye sahip hastalarla karşılaştırıldığında tümör gradeleri, myometrial invazyon derinlikleri, lenf nodu tutulumları, histolojik subtipleri ve hasta yaşları açısından benzer dağılım göstermekteydi. Pozitif sitoloji rekürrens riskini artırma eğilimine neden olmaktaydı. Klinik erken evre endometrium kanserinde pozitif sitoloji bağımsız bir prognostik faktör olup, taşıdığı kötü. prognoz, yalnızca diğer olumsuz faktörlerin bir yansıması değildir.
