Abstract

OBJECTIVE: To investigate the effect of Doderlein bacillus on the other cervico-vaginal cytologic findings and, to determine relationship between Doderlein bacillus and some cliniçal parameters. STUDY DESIGN: Totally 614 refered women who have well-investigated cervico-vaginal cytologic examination were included in the study. Two hundred eighty-two cases in which Doderlein bacillus was detected on cervico-vaginal cytologic examination were taken as a study group, the remaining cases were taken as a control group. RESULTS: Doderlein bacillus was detected in 282 (45.9%) cases on cervico-vaginal smears. Epithelial cells that show evidence of infection were found significantly lower rate in Doderlein bacillus detected smears (7.8%) than in non-detected smears (27.4%, p<0.000). Also other cytologic findings related to pelvic infection were found significantly lower rate in Doderlein bacillus detected smears than in non-detected smears. Among 39 atrophic cervico-vaginal smears, Doderlein bacillus was detected only in one case (p<0.000). CONCLUSION: Doderlein bacillus was not present on cervico-vaginal smears of more than half of the cases (54.1%). So there should be other mechanisms except than Doderlein bacillus and acid vaginal environment for protection against the cervico-vaginal infections. If there Doderlein bacillus was not present on cervico-vaginal cytologic examination two situations should be excluded; 1. infection 2. atrophy. Because many systemic antibiotics and vaginally applied drugs may reduce growth of Doderlein bacillus, widespread abuse of antibiotics should be prevent. (Gynecol Obstet Reprod Med 1997; 3:355-360)
