Abstract
OBJECTIVE: To report our experience with medically inoperable Stage I-II endometrial carcinoma treated with radiation alone. STUDY DESIGN: Between January 1990 and December 1997, twenty patients with endometrial carcinoma having medical risks precluding surgery received high dose rate brachytherapy (HDRB) either with or without external beam radiotherapy. All patients were clinically staged according to the criteria of FIGO, 1971. Patients with Stage I disease were given three fractions of HDRB with one week intervals, where the fraction dose was 1000 cGy. Patients with Stage II disease were treated with external beam radiotherapy plus HDRB. External beam radiotheraphy consisted of whole pelvic irradiation with Co 60 employing opposed AP-PA fields with a mean external beam dose of 4600 cGy using 200 cGy per fraction. The fraction dose of HDRB was between 850-1000 cGy, for patients with Stage II disease. After completion of treatment, all patients were followed up every three months for the first two years and every six months, thereafter. Response to treatment was evaluated by pelvic examination, cervical cytology and pelvic sonography. Endometrial biopsy was obtained when recurrence was suspected. Actuarial survival and disease specific survival rates were calculated by using Life-Table. RESULTS: The mean age of the patients included in the study was found to be 60 years (46-80). Four women had Stage I disease (20%) whereas 16 patients had Stage II disease (80%). Acturial 2-year, 3-year and 5-year survival rates were found to be 89.4%, 71.5% and 71.5%, respectively for all patients included in the study. The 2-year and 5-year disease specific survival rates were found to be 84.6% and 56.4% respectively for patients with stage II disease. Two patients with Stage II disease (12.5%) had pelvic recurrences. There were no acute complications related with radiation therapy. Two patients (10%) had grade II large bowel complications in the follow-up period. CONCLUSION: Although HDRB with or without external beam radiotherapy is safe and effective for stage I and II endometrial carcinoma, radiation therapy alone should only be considered when the risk of surgical intervention is high for the patient.

Amaç: Medikal nedenlerle opere edilemeyen Evre I-II endometrium kanseri olgularında primer radyoterapi ile tedavi sonuçlarını bildirmek. Çalışma Şekli: Ocak 1990-Aralık 1997 tarihleri arasında cerrahi girişime engel teşkil eden medikal riskler nedeniyle opere edilemeyen 20 endometrium kanseri olgusuna ait klinik veriler retrospektif olarak incelendi. Tüm olgulara FIGO 1971 kriterlerine göre klinik evrelendirme yapıldı. Evre l olgulara bir hafta arayla üç fraksiyon (fraksiyon dozu: 1000 cGy) yüksek doz brakiterapi (high dose rate brachytherapy: HDRB) uygulandı. Evre II olgulara eksternal pelvik radyoterapi ile birlikte HDRB uygulandı. Tedavi bitiminde olgular ilk iki yıl sürecince üç ayda bir, daha sonra altı aylık intervallerle izlendiler. Tedaviye yanıt; pelvik muayene, servikal sitoloji ve pelvik sonografi ile değerlendirildi. Rekürrens şüphesinde endometrial biopsi yapıldı. Yaşam tablosu kullanılarak hastalığa özgü sağkalım oranı ve beş yıllık sağkalım oranı hesaplandı. Bulgular: Çalışma kapsamındaki olguların ortalama yaşının 60 (48-80) olduğu belirlendi. Dört olgu Evre l(%20), 16 olgu Evre II (%80) endometrium kanseri tanısı aldı. Tüm olgular için iki, üç ve beş yıllık sağkalım oranları sırasıyla %89.4, %71.5 ve %71,5 olarak belirlendi. Evre II olgular için iki ve beş yıllık hastalığa özgü sağkalım oranları sırasıyla %84.6 ve %56.4 olarak bulundu, izlem süresince, Evre II olgulardan ikisinde pelvik rekürrens saptandı (%12.5). Radyoterapi ile ilgili akut komplikasyon izlenmedi, iki olguda (%10) izlem süresince Grade II kalın barsak komplikasyonu geliştiği tespit edildi. Sonuç: Evre l ve II endometrium kanserinde eksternal radyoterapi ile kombine HDRB ya da tek başına HDRB güvenli ve etkin bir tedavi şekli olsa da tedavi seçeneği olarak primer radyoterapi, sadece hasta için cerrahi girişim riskinin çok yüksek olduğu hallerde düşünülmelidir.
