Abstract
We present a case of placenta increta treated conservatively with methotrexate. A 21 year old, gravida 2, para 1, abortion 1 patient was seen postpartum with an adherent placenta. She was given 1 mg/kg weekly methotrexate injections and the therapy was monitored with serum creatine kinase, beta-human chorionic gonadotropin and alpha-fetoprotein measurements. Periodic ultrasonographic evaluations showed complete resorption of the retained placenta 3.5 months later. We conclude that methotrexate is a promising choice of treatment in hemodynamically stable patients who desire fertility. Those patients must be followed-up closely for infection and hemorrhage
Özet
Methotrexate ile konservatif olarak tedavi edilen bir plasenta inkreata olgusu sunuyoruz. Doğum sonrası yapışık plasenta nedeniyle değerlendirilen 21 yaşındaki olgunun 2 gebelik 1 doğum 1 düşük öyküsü vardı. Haftalık 1 mg/kg methotrexate enjeksiyonları yapıldı ve serum kreatinin kinaz, beta human koryonik gonadotropin ve alfa-fetoprotein ölçümleri ile olgu monitörize edildi. Ultrasonografik kontroller 3,5 ay sonra kalıntı plasentanın tamamen rezorbe olduğunu göstermiştir. Fertilite isteği olan hemodinamik açıdan stabil olgularda methotrexate'ın umut veren bir tercih olabileceği sonucuna vardık. Bu hastalar enjeksiyon ve hemoraji açısından yakın takipte olmalıdırlar.
