The reasons for intrauterine device discontinuation and subsequent contraception

Yaşları 15 ile 44 arasında değişen, daha önce RİA kullanmış, konsepsiyon amacıyla - kişisel veya yan etkilere bağlı olarak yöntemi bırakmış olan 228 kadın çalışmaya alınmıştır. Rahim içi araç (RlA) kullanıcılarında, yöntem tercih-bırakma sebeplerini incelemek ve bir sonraki kontraseptif yöntem dağılımını belirlemek amacıyla çalışmayı planladık. Kadınların başlangıçta RİA tercih etme sebepleri sıklıkla, yöntemin etkinliği(%43.70) ve kullanım kolaylığı (%35.43) idi. Kadınların RİA bırakma sebeplerine baktığımızda kişisel sebeplerin ön sırada olduğunu görmekteyiz. Komplikasyonlardan dolayı yöntem bırakanlarda ise; enfeksiyon + vajinal akıntı (%19.74), vajinal kanama (%17.54) ve pelvik ağrı(%15.79) sıklıkla karşılaşılan yönteme bağlı yan etkiler idi. RİA'ı bırakan bu kadınlar daha sonraki dönemde, en sık RİA (% 28.70), geri çekme (% 22.81) ve kondom (% 21.05) yöntemlerini tercih etmişlerdir. Ancak RİA bırakma sebepleri ile bir sonrrki kontraseptif yöntem dağılımı arasında anlamlı bir ilişki tesbit edilememiştir. Sonuç olarak, RİA herhangi bir sebepten dolayı yöntemi bırakanlarda bir sonraki kontraseptif yöntem olarak yine en sık kullanılan yöntemdir. Bunda RİAnın etkin koruma sağlayışmın ve kullanım kolaylığının rolü büyüktür.
Abstract
OBJECTIVE: To determine the factors which attract the users for intrauterine devices, reasons for discontinuation and subsequent contraceptive methods used. STUDY DESIGN: Two hundred and twenty-eight parous women who had copper intrauterine device (IUD) removed either to permit conception or because of complications and personal causes were studied. Ouestionaire was applied to determine factors which attract IUD at the beginning or discontinue intrauterine device and subsequent preferences for fertility regulation were determined. Then, the relationship between the discontiuation reasons and subsequent contraception were searched. RESULTS: Most of the women used intrauterine device due to high efficacy (43.70%) and once only approach to contraception (35.43%). The most common discontinuation reasons were personal factors (32.46 %). Side effects such as infection (19.74%), vaginal bleeding (17.54%) and pelvic pain.(15.79%) were less common factors. The most commonly used subsequent contraceptive methods were intrauterine device (28.07%), coitus interruptus (22.81%) and condom (21.05%) .There was no statistically significant relationship between the discontinuation reasons and subsequent contraception (p>0.05). CONCLUSION: IUD was the mostly preferred contraceptive method as a subsequent contraception, due to effectiveness of the method and once only approach to contraception.
