Abstract
OBJECTIVE: The purpose of this study was to evaluate the role of splenectomy in the cytoreductive surgery for advanced ovarian carcinoma. STUDY DESIGN: Retrospective analysis was performed on the data obtained from the files of gynecologic oncology unit. Between May 1996 and May 1998, 12 patients underwent splenectomy during cytoreductive surgery for epithelial ovarian carcinoma. RESULTS: An optimal cytoreduction was achieved in 8 patients. The indications for splenectomy were, in descending order, tumoral involvement in the splenic hilus and/or capsule, facilitation of total omentectomy in the presence of perisplenic disease and iatrogenic injury of the spleen. Six patients had splenic capsular involvement. The postoperative complications were usually of pulmonary origin. CONCLUSIONS: Splenectomy was found to be a relatively safe component of cytoreductive surgery.
Özet
Bu çalışmanın amacı, ileri evre över kanseri olgularında, primer Sitoredüktif cerrahi kapsamında splenektominin etkinliği ve güvenilirliğinin belirlenmesidir. Mayıs 1996 - Mayıs 1998 tarihleri arasında Sitoredüktif cerrahi esnasında splenektomi gerçekleştirilen 12 olgunun verileri retrospektif olarak değerlendirilmiştir. Bu olgularda splenik hilus ve/veya parankim tutulumu nedeniyle, perisplenik hastalık varlığında omentektominin kolaylaştırılması amacı ile veya dalağın iatrojenik yaralanması nedeniyle Sitoredüktif cerrahide splenektomiye ihtiyaç duyulmuştur. Kliniğimizde, bizzat jinekolojik onkoloji ekibi tarafından gerçekleştirilen operasyonlarda elde edilen tecrübeler literatür bilgileri ışığında değerlendirilerek, splenektominin Sitoredüktif cerrahideki yeri, getirdiği avantaj ve dezavantajlar tartışmaya sunulmaktadır.
