Abstract
OBJECTIVE: To asses the effectiveness of laparoscopic drilling for induction of ovulation in resistant polycystic ovarian syndrome. STUDY DESIGN: Eighteen women with the PCOS, complaining of anti-estrogen resistant anovulatory infertility, entered the study during a period of twelve months. Mean age of women was 28 (24-31 years of age). Duration of infertility was between 21-49 months (mean 34,2 months). Laparoscopic ovarian drilling was performed via direct trocar insertion through umblicus and using carbondioxide as insuflation media. 6-10 holes were burned in each ovarian capsule via monopolar electrocautery. Following the operation, ovulatory status was monitored by ultrasonography and by progesterone estimation on blood samples obtained 21th day of menstruation (> 8 ng/dl). All except three patients underwent second look laparoscopy in the third month of primary endoscopy. RESULTS: After laparoscopic ovarian drilling ovulation was obtained in 83% of the patients. 9 of the fifteen patients who gained spontaneous ovulatory cycles became pregnant within one year following the operation. CONCLUSİON: Laparoscopic ovarian drilling is an effective way of treatment for the anti-estrogen resistant anovulatory infertility in women with the PCOS.

Rezistan polikistik over sendromlu hastalarda ovulasyon indüksiyonu için laparoskopik ovaryen drillingin önemi araştırıldı. Anti-östrojen tedaviye dirençli 18 PCOS'lu olgu çalışmaya dahil edildi. Her olguda laparoskopik drilling, her iki over korteksine monopolar elektrokoter ile 6-10 delik açılarak yapıldı. Operasyon sonrası ovulasyon durumu ultrasonografi ve kan progesteron düzeyi ile takip edildi. Bir yıllık takip sonrasında.onsekiz olgunun onbeşinde (%83) ovulasyon ve dokuzunda gebejik 'elde edildi. Medikal tedaviye cevap vermeyen PCOS 'lu olgularda laparoskopik ovaryen drillingin etkili bir tedavi yöntemi olduğu gösterildi.
