Abstract
OBJECTIVE: To evaluate effectiveness of clomiphene citrate (CC) and prednisone in 25 CC resistant patients in a retrospective review. STUDY DESIGN: Twenty five infertile women who failed to ovulate at a dosage of CC 150 mg over 5 days received 150 mg of CC on days 5 to 9 with 5 mg prednisone at the hour of sleep throughout the cycle. Ovulation was determined by ultrasound criteria and a rise in midluteal progesterane37 ng/ml. Clinical pregnancy was determined by serum HCG levels and transvaginal ultrasound. RESULTS: Seventeen (68%) of 25 patients had ovulatory and 4 of 25 patients (16%) conceived with this therapy. There were no cases of ovarian hyperstimulation and any side effects in this study. CONCLUSIONS: A combination of CC plus prednisone can be used when a reasonable trial of CC alone has failed to achieve normal ovulation.

Kiomiîen sitrata direnç gösteren 25 haftada kiomifen sitrat ve prednisonun etkisini retrospektif olarak değerlendirmek. Beş günden fazla günde 150 rng kiomifen sitrat aldığı halde ovulasyon gerçekleşmeyen 25 infertil kadına 5 ve 9. Günler arasında 150 mg klomifen sitrat ile birlikte gece 5 mg prednison sıklus boyunca verildi. Ovulasyon, ultrasound kriterleri ve midiuteal progesteronun >= 7 ng/ml olması ile belirlendi. Klinik gebelikse serum BHCG düzeyi ve transvagina! ultrasound ile değerlendirildi. Yirmibeş hastadan 17 sinde (%68) ovulasyon oldu. 4 hastada ise (%16) gebelik gerçekleşti. Hiç ovarian stimulasyon ve diğer yan etkiler gözlenmedi. Klomifen sitratın tek başına ovulasyon oluşturamadıği durumlarda klomifen sitrat ve prednizon kombinasyonu kullanılabilir.
