Abstract
OBJECTIVE: To evaluate the results of the surgical management of large dermoid cysts by laparoscopy-assisted cystectomy. STUDY DESIGN: In a retrospectively designed study, we evaluated the results of the surgical management of 16 large ( >8 cm) benign dermoid cysts by laparoscopy-assisted cystectomy between January 1996 and August 1998. In all patients, after suctioning the cyst fluid, the ovarian capsule or cyst was taken through the trocar site out of the abdominal cavity by enlarging the skin incision in order to identify the functional ovarian tissue and complete the cystectomy. We assessed the duration of hospitalization, operative time, post-operative morbidity. RESULTS: All patients were managed successfully by laparoscopy. The mean diameter of the cysts was 10.5 cm ( range 8-17 cm).The mean operative time was 65.5 minutes ( range 45-95 minutes ) and the mean hospital stay was 1.7 days ( range 1-3 days ). The leakage of dermoid contents into the peritoneal cavity occurred in all cases and managed by copious lavage. None of the patients developed post-operative morbidity associated with chemical peritonitis due to the cyst content. CONCLUSION: Laparoscopy-assisted cystectomy is a safe and effective technique in the management of large dermoid cysts (>8 cm).

Bu çalışmanın amacı, büyük dermoid kistlerin laparoskopi-asiste kistektomi ile cerrahi tedavisinin sonuçlarını değerlendirmektir. Ocak 1996 ile Ağustos 1998 yılları arasında laparoskopi-asiste kistektomi uygulanmış 8 cm den büyük çaplı 16 dermoid kistin cerrahi tedavi sonuçları retrospektif olarak incelenmiştir. Bütün hastalarda över kapsülü veya kist, fonksiyonel över dokusunu tanımlamak için ve kistektomiyi tamamlayabilmek için trokar bölgesindeki cilt insizyonunu genişleterek abdominal kavite dışına alınmıştır. Bu prosedürde, hospitalizasyon süresi, operasyon süresi, postoperatif morbidite değerlendirilmiştir. Bütün hastalarda prosedür başarı ile uygulanmıştır. Ortalama kist çapı 10.5 cm (8-17 cm), ortalama operasyon süresi 65.5 dakika (45-95 dk. ) ve ortalama hospitalizasyon süresi 1.7 gündür (1-3 gün). Bütün vakalarda dermoid içeriği peritoneal kaviteye yayılmıştır ve yoğun yıkama ile tedavi edilmiştir. Hastaların hiçbirisinde kist içeriğine bağlı kimyasal peritonite bağlı postoperatif morbidite görülmedi. Sonuç olarak, laparoskopi-asiste kistektomi büyük dermoid kistlerin tedavisinde güvenilir ve etkili bir tekniktir.
