Abstract
Fetal cystic hygromas as a manifestation of malformation of lymphatic system are often the only sign of an severe fetal anomaly. Most of cases with cystic hygroma is associated with Turner syndrome (45.X). We presented a case of cystic hygroma occuring in a twin pregnancy, and discussed the management of this condition. A 25-year-old woman, was referred for prenatal care at 15 weeks of gestation. Ultrasound revealed one twin to have cystic hygroma and the other twin appeared normal. After counselling, the parents decided to terminate the fetus with cystic hygroma. The fetocide with a single puncture amniocentesis was done by intracardiac potassium chloride injection performed to the cystic hygroma. With an another puncture amniotic fluid was taken from the remaining twin. Karyotypes showed 45, XO for the cystic hygroma and 46, XY for the other fetus.A male infant weighing 2400 gr was born at 33 weeks gestation in a spontaneous vaginal delivery. No problem was noted on physical examination.

Lenfatik sistem malformasyonlarının bir sonucu olan fetal kistik higroma, şiddetli fetal bir anomalinin sıklıkla tek bulgusudur. Kistik higromaların çoğu Turner sendromu (45,X) ile birliktedir. Biz ikiz gebelik esnasında gelişen bir kistik higroma olgusunu sunup tedavisini tartıştık. Yirmi beş yaşındaki hasta gebeliğin 15.haftasında prenatal takip için başvurdu. Ultrasonografik incelemede; biri normal, diğerinde kistik higroma gelişimi olan ikiz gebelik saptandı. Görüşme sonrası, ebeveynler anomalili fetus için gebeliğin sonlandırılmasına karar verdiler.Amniosentez ile kistik higromalı fetusa intrakardiak potasyum klorür injeksiyonu yapıldı.Diğer fetustan başka bir iğne ile amniyotik sıvı alındı. Karyotip kistik higroma için 45.XO , diğer fetus için 46.XY idi. 33.haftada spontan vajinal yolla 2400 gr ağırlığında erkek infant doğurtuldu. Fizik incelemede herhangi bir sorunla karşılaşılmadı.
