Abstract

OBJECTIVE: To evaluate whether a decrease in RBC deformability occurs in severe preeclampsia, which is not associated with a growth-retarded fetus, by using computerized cell transit analyser (CTAX). STUDY DESIGN: Four ml of arterial and venous blood, anticoagulated by K-EDTA, was withdrawn from 11 severe preeclamptic women qith appropriately-grown fetuses and 16 normotensive healthy pregnant women of similar gestational ages chosen randomly. We used two different sources of blood to investigate whether a difference in PO2 saturation cause a RBC deformability change since hemoglobin O2 concentrations could be effective on hemoglobin solubility and consequently deformability. Erythrocyte deformability was evaluated by cell transit analyser (CTA). RESULTS: Mean hematocrit (htc) value of preeclamptic patients was 39,70,81 whereas control group's was 32,330,6 (p<0.05). Mean RBC transit times measured by CTA were 3,020,37 and 3,050,28 milliseconds (msecs) in venous and erterial samples respectively for the preeclamptic patients and 2,960,31 and 2,990,47 msecs for the healthy controls which were not significantly different. Both MCV and MCHC of erythrocytes in preeclamptic patients and controls were not found different. CONCLUSIONS: RBC deformability was reduced significantly in preeclampsia/ecclampsia associated with FGR. We found longer RBC transit times in the study group than in healthy controls but the difference was not statistically significant. The relation between RBC deformability and FGR needs to be further clarified as we tried with this special subgroup of preeclamptic patients in this study. (Gynecol Obstet Reprod Med 2001; 7:82-85)

Bu çalışmanını amacı IUGG olmayan şiddetli preeklampsi olgularında eritrosid deformabilitesinde değişiklik olup olmadığını kompüterize cell transit analyser kullanarak araştırmaktır. Elektronmikroskop ile yapılan çalışmalarda da eritrositlerin içeriklerinde ve morfolojilerinde meydana gelen değişiklikler gösterilmiştir. Bu tür membran değişiklikleri mikroanjiopatik hemolize yatkınlık yaratabilir. Bu tür membran değişiklikleri sonucu eritrosit deformibilitesinin değişimi, mikrosirkülasyonda eritrositlerin aşırı deformasyonuna ve oksijen taşıma özelliklerinin değişmesine etkili olabilir. 11 uygun gelişim gösteren şiddetli preeklampsi olgusu ve 16 normotensive sağlıklı kontrol olgusunda 4 ml arteriel ve venöz kan alındı. İki farklı kaynaktan alınan bu kan örnekleri CTA ile eritrosit deformibilitesi yönünden değerlendirildi. Preeklamptik olgulardaki ortalama hematokrit değeri 39,70,81 olarak bulunmuş ve kontrol grubun da ise 32,33 0,6 (p<0.05). CTA ile ölçülen ortalama eritrosit geçiş süresi preeklamptik olguların venöz ve arteriel kan örneklerinde 3,02 0,37 ve 3,050,28 milisaniye (msn) ve kontrol gurubunun arteriel ve venöz kan örneklerinde ise 2,960,31 ve 2,990,47 msn olarak ölçülmüş ve istatistiksel olarak anlamlı bulunmamıştır. Preeklamptik olgularda ve kontrol gurubunda MCV and MCHC değerleri de farklı bulunmamıştır.
