Abstract
OBJECTIVE: Acute appendicitis in pregnancy is the most common non-obstetric complication warranting emergency laparotomy. The goal of this study is the evaluation of clinical manifestations, diagnosis and management of acute appendicitis in pregnancy. STUDY DESIGN: 25 pregnant patients to whom was laparotomy performed for suspected acute appendicitis in Ankara Numune Education and Research Hospital between 1995-1999 were evaluated retrospectively. RESULTS: In 16 (64%) cases ,acute appendicitis was proven at laparotomy and by subsequent histopathology. In the confirmed cases, 4 (25%) cases in the first trimester, 10 (62.5%) cases in the second and 2 (12.5%) cases in the third trimester. Abdominal pain was the most common presenting symptom, the second was vomitting. Abdominal tenderness and rebound tenderness were the most common physical signs, although the latter was less marked in late pregnancy. Laparotomy was performed within the first 24 hours of onset of symptoms in all patients. Perforation of appendix was found in 2(12.5%) patients. Pregnancy complications were found to be considerable; 2/4 (50%) who underwent appendectomy for appendicitis in the first trimester aborted spontaneously. However, no pregnancy complications were observed following second and third trimester appendectomies. In our study , there were no fetal and maternal death. CONCLUSION: Acute appendicitis in pregnancy is the most common non-obstetric surgical emergency. Appendicitis in pregnancy should be suspected when a pregnant women complains of new abdominal pain. No laboratory finding was found to be diagnostic for acute appendicitis during pregnancy. The combination of symptoms and clinical judgement is still important in deciding to laparotomy. We think that early suspicion and serial examination in pregnancy may result in appropriate interventions for appendicitis.

Akut apandisit gebelikte en sık acil laparotomiye neden olan non-obstetrik komplikasyondur. Bu çalışmada, gebelikte görülen akut apandisitin kliniği,teşhisi ve tedavisinin araştırılması amaçlanmıştır. Ankara Numune Eğitim ve Araştırma Hastanesinde 1995-1999 yılları arasında akut apandisit tanısıyla laparotomi yapılan 25 gebe kadın retrospektif olarak değerlendirildi. 16 (%64) olguda,akut apandisit laparotomi ve ardından histopatolojik olarak kanıtlandı. Bu olgulardan 4(%25)'ü birinci trimester, 10(%62.5)'u ikinci trimester ve 2(%12.5)'i üçüncü trimesterdeydi. En sık görülen semptom karın ağrısıydı,bulantı ikinci sırada yer aldı.Abdominal hassasiyet ve rebound en sık rastlanan fizik muayene bulgularıydı.Tüm hastalara şikayetlerin başlamasından itibaren 24 saat içinde laparatomi yapıldı.2(%12.5) olguda perforasyon görüldü.İlk trimesterdeki 4 olgudan 2(%50)'sinde komplikasyon olarak spontan abortus gelişti.İkinci ve üçüncü trimester olgularında komplikasyon görülmedi.Çalışmamızda fetal ve maternal ölüm olmadı. Akut apandisit, gebelikte en sık görülen ve cerrahi tedavi gerektiren non-obstetrik acil durumlardan biridir.Gebe kadın karın ağrısından şikayet ediyorsa akut apandisit akla getirilmelidir.Gebelikte,akut apandisit tanısını kolaylaştıracak bir laboratuar bulgusu yoktur. Semptomlar ve klinik tecrübe hala laparatomi kararını verdiren en önemli kombinasyondur. Erken şüphe ve seri muayenenin gebelikte akut apandisit tanısını koyma ve tedaviye başlamada en önemli unsur olduğunu düşünmekteyiz.
