Abstract

OBJECTIVE: The aim was to identify risk factors for wound infection in patients who receive antibiotic prophylaxis before abdominal hysterectomy for benign conditions. STUDY DESIGN: The prospective study included 61 women who underwent abdominal hysterectomy for benign conditions. All patients received an intravenous dose of 2 g first-generation cephalosporin before their operation. Pfannenstiel's incisions were used in all cases, and the two surgeons who performed the procedures used standard surgical technique. Patients who required suction drains or had suspected infection prior to surgery were excluded. RESULTS: Wound infection was diagnosed in 5 of the 61 patients (8.2%). Body weight (P=0.002), body mass index (P=0.006), and duration of operation (P=0.025) were statistically associated with wound infection. Logistic regression analysis confirmed duration of operation as the only significant risk factor for wound infection (P=0.02) (odds ratio 1.04; 95% confidence interval 1.006-1.08). Four of the five patients who developed infection had operation times longer than 115 minutes. CONCLUSION: Duration of surgery is a significant risk factor for wound infection in women who receive antibiotic prophylaxis prior to abdominal hysterectomy.

Benign nedenlerle abdominal histerektomi yapılan ve preop antibiyotik proflaksisi alan hastalarda yara enfeksiyonuna sebep olabilecek risk faktörlerini belirlemeyi amaçladık. Bu prospektif çalışmaya dahil edilen 61 hastanın hepsine operasyon öncesi 2 gr i.v birinci kuşak sefalosporin uygulandı. Tüm hastalara, iki cerrah tarafından pfannenstiel insizyonunu takiben standart cerrahi yöntemler uygulandı. Cerrahi öncesi enfeksiyon şüphesi olan veya operasyonda dren konması gereken hastalar çalışmaya alınmadı. 61 hastanın 5'inde (% 8.2) yara enfeksiyonu geliştiği tespit edildi. Vücut ağırlığı (p= 0.002), vücut kütle indeksi (p=0.006),ve operasyon süresi (p= 0.025) ile yara enfeksiyonu arasında istatistiksel olarak anlamlı ilişki saptandı. Logistik regresyon analizi sonucunda operasyon süresi, yara enfeksiyonu açısından tek önemli risk faktörü olarak belirlendi (p=0.02)(odds ratio 1.04; %95 confidence interval 1.006-1.08). Yara enfeksiyonu gelişen beş hastanın dördünde operasyon süresi 115 dakikadan daha uzundu. Antibiyotik proflaksisi altında abdominal histerektomi yapılan hastalarda, operasyon süresi, yara enfeksiyonu açısından önemli bir risk faktörüdür.
