Abstract
OBJECTIVE: The purpose of our study was to investigate whether maternal serum levels of the natural anticoagulants (Protein C, Protein S, and antithrombin III) in preeclampsia differ from normal pregnant women and whether hypercoagulability could be a contributory factor in the pathogenesis of preeclampsia. STUDY DESIGN: Serum levels of Protein C, Protein S and antithrombin III were measured in 40 patients in the third trimester of pregnancy with documented preeclampsia and 20 healthy normotensive women with similar maternal and gestational ages. Mann Whitney-U and Chi-square tests were used for statistical analysis. p<0.05 was regarded as statistically significant. RESULTS: The mean serum levels of Protein C were 103.95% versus 109.60% and the mean serum level of Protein S was 106.75% versus 116.49% in the study and the control groups. The differences between the groups were not statistically significant (p=0.263) and (p=0.310) respectively. The mean serum level of antithrombin III was 92.43% versus 104.50% in the study group with preeclampsia and the control group. The difference between the study group and the control group was statistically significant (p=0.008). CONCLUSION: Hypercoagulability caused by the natural anticoagulant antithrombin III deficiency could be a contributory factor in the pathogenesis of preeclampsia

Normal gebelerle preeklampsi tanısı konmuş gebelerin serum doğal antikoagulan (protein C, protein S, antitrombin III) seviyelerini kıyaslamak ve doğal antikoagulanların eksiklikleri nedeniyle oluşabilecek hiperkoagulabilitenin preeklampsi etiolojisinde bir etken olup olmadığını araştırmak amaçlandı. Benzer yaş ve gestasyonel yaştaki 40 preeklampsi tanısı konmuş gebe ile üçüncü trimesterde ki 20 normal gebenin protein C, protein S ve Antitrombin III seviyeleri ölçüldü. Mann Witney U ve Ki kare testi kullanılarak veriler arasında anlamlı bir fark olup olmadığı araştırıldı. P<0.05 değeri anlamlılık sınırı olarak kabul edildi. Çalışma grubunu oluşturan preeklampsi tanısı konmuş gebelerin ortalama protein C düzeyi %103.95, protein S düzeyi %106.75 olup, kontrol grubunda bu değerler sırayla % 109.60, %116.49 olarak ölçülmüştür. İki grup arasında istatistiksel olarak anlamlı fark mevcut değildir (p=0.263, p=0.310). Ortalama serum antitrombin III seviyeleri ise çalışma grubunda %92.43, kontrol grub unda %104.50 olarak ölçülmüştür ve arada istatistiksel olarak anlamlı fark mevcuttur (p=0.008). Düşük serum antitrombin III seviyeleri neticesinde meydana gelebilecek hiperkoagülabilite preeklampsi patogenezinde bir etken olabilir.
