The results of exchange surgery in the eyes with opacified intraoculer lens

Abstract
Purpose: To evaluate the clinical outcome and complications of exchange surgery in eyes with opacified hydrophilic acrylic intraocular lenses (IOLs). Materials and Methods: Twenty - eight eyes of 22 patients which that had been implanted with the same brand of IOL (Intraocular Optical International, California) underwent a exchange surgery because of significant deterioration of vision related to the calcific opacification. Intraoperative and postoperative complications were evaluated. Results: In the 17 eyes opacified IOLs were explanted totally, and in 9 acrylic foldable IOLs and in 8 polymethylmethacrylate IOLs were implanted.. The opaque acrylic IOLs could only be removed with the entire capsule in 3 eyes. In the remaining 8 eyes IOLs were explanted by cutting the optic from the haptics followed by implantation of hydrophobic acrylic IOLs in the ciliary sulcus. Intraoperative complications were zonular dialysis in 4 eyes and posterior capsular tears in 4 eyes. Mean preoperative and postoperative best corrected visual acuity (BCVA) (decimal) values were 0.0963 ± 0.0689 SD (range 0.016-0.3) and 0.64 ± 0.26 SD (range 0.2-1.0), respectively. The visual improvement was statistically significant (Wilcoxon sign rank test p<0.0001). Conclusions: IOL exchange surgery for opacified IOLs can yield a significant improvement in the visual function of patients. However, the ophthalmic surgeon should carefully analyze the concordance between the density of IOL opacification, the preoperative visual acuity, and co-morbidity of the eye. Moreover, the complaints and vision needs of the patient should also be considered.

Amaç: Kesifleşmiş hidrofilik akrilik göz içi merceği (GİM) olan gözlerde değişim cerrahisinin klinik sonuçları ve komplikasyonlarını değerlendirmek. Gereç ve Yöntem: Aynı marka (Intraocular Optical International, California) GİM implante edilen 22 hastanın 28 gözü, kalsifik kesifleşmeye bağlı anlamlı görme kaybı nedeniyle değişim cerrahisine gitti. Ameliyat sırasındaki ve sonrası komplikasyonlar değerlendirildi. Bulgular: Onyedi gözde kesifleşmiş lensler tamamıyla çıkarıldı, 9’una katlanabilir akrilik ve 8’ine polimetilmetakrilat lens yerleştirildi. 3 gözde kesif akrilik GİM’ler ancak kapsülle beraber çıkarılabildi. Kalan 8 gözde optik kısmın haptiklerden kesilerek çıkarılmasını siliyer sulkusa hidrofobik akrilik GİM yerleştirilmesi izledi. Ameliyat içi komplikasyonlar 4 gözde zonül dializi ve 4 gözde arka kapsül yırtığı idi. Ortalama ameliyat öncesi ve sonrası en iyi düzeltilmiş görme keskinlikleri (EDGK) (ondalıklı) sırasıyla 0.0963±0.0689 (0.016-0.3 arası) ve 0.64±0.26 (0.2-1.0 arası) olarak bulundu. Görsel iyileşme istatistiki olarak anlamlıydı (Wilcoxon signed Ranks Test p<0.0001). Sonuç: Kesifleşmiş GİM’lerde, değişim cerrahisi hastaların görsel fonksiyonlarında anlamlı artış sağlayabilir. Bununla birlikte göz cerrahı, GİM kesifliğinin yoğunluğu, ameliyat öncesi görme keskinliği ve gözdeki diğer hastalıklarla arasındaki ilişkiyi iyi değerlendirmelidir. Bunun da ötesinde hastanın şikayetleri ve görsel ihtiyaçları da dikkate alınmalıdır.
