Spheric refractive shift induced by different type of intraocular lenses after phacoemulsification surgery

Abstract

Purpose: We investigated postoperative spherical refractive changes in patients undergoing phacoemulsification with different types of intraocular lens implantation. Materials and Methods: Three hundred eighty seven patients undergoing phacoemulsification between May 2000 and May 2002 were included in the study. Types and number of implanted IOLs were: AR40 (hydrophobic acrylic /Allergan) in 102 eyes, SI40 (Silicon/ Allergan) in 68 eyes, Acrysof (hydrophobic acrylic / Alcon) in 57 eyes, CeeOn (silicon /Pharmacia) in 28 eyes, Softec (hydrophilic acrylic/Lenstec) in 72 eyes, and PS52 (PMMA, Allergan) in 60 eyes. Dioptries of IOLs were calculated with SRK II formula using A scan biometry (Mentor, USA). Differences between autorefractometry values measured preoperatively and postoperatively after one month were determined. Refractive shifts obtained for different IOL types were statistically compared using analysis of variance (ANOVA) and posthoc test. Results: The refractive shifts determined were as following: -0.53 D in AR40 group, -0.43 D in SI40 group, -0.27 D in Acrysof group, -0.83 D in Softec group, -0.56 D in CeeOn group, and +0.26 in PMMA group. Refractive change was between -3.00 and +2.00 D. There was no statistically significant difference among the IOL groups except PMMA group. Conclusion: All of the IOL types studied have similar spherical refractive change after uneventful phacoemulsification with in the bag IOL implantation. Although Acrysof induced less spherical refractive change, all IOL types except PMMA caused relatively small and reasonable myopic shift compared with preoperative spherical refraction.

Amaç: Fakoemülsifikasyon cerrahisi ile farklı tipte göziçi lens implantasyonu yapılmış olan gözlerde cerrahi sonrası ortaya çıkan sferik refraktif kaymanın değerlendirilmesidir. Gereç ve Yöntem: Mayıs 2000 ve Mayıs 2002 yılları arasında fakoemülsifikasyon cerrahisi uygulanan 387 hasta çalışmaya dahil edildi. GİL dioptrileri A scan biometri (Mentor,USA) cihazında SRK II formülü ile ölçüldü. Kapsüloreksis ve fakoemülsifikasyon işleminden sonra 102 göze AR40 (hydrophobic acrylic /Allergan), 68 göze SI40 (Silicon/ Allergan), 57 göze Acrysof (hydrophobic acrylic / Alcon), 28 göze CeeOn (silicon /Pharmacia), 72 göze Softec (hydrophilic acrylic/Lenstec) ve 60 göze ise PS52 (PMMA, Allergan) kapsül içi olarak implante edildi. Ameliyat öncesi ve ameliyat sonrası 1. ayda ölçülen otorefraktometre değerleri arasındaki farklar tespit edildi. Farklı GİL tipleri için elde edilen sferik refraktif kayma değerleri varyans analizi (ANOVA)ve posthoc testi ile istatistiksel olarak değerlendirildi. Bulgular: Ameliyat sonrası sferik refraktif kayma AR40 grubunda -0.53 D, SI40 grubunda -0.43 D, Acrysof grubunda -0.27, Softec grubunda -0.83, Ceeon grubunda -0.56 D ve PMMA grubunda ise +0.26 D idi. Refraksiyon değişimi -3.00 D ile +2.00 dioptri arasındaydı. PMMA grubu haricindeki diğer GİL tipleri arasında sferik refraktif kayma açısından anlamlı fark yoktu. Sonuç: Sorunsuz fakoemülsifikasyon cerrahisi ve kapsül içi implantasyon sonrası tüm GİL tiplerinde ameliyat sonrası benzer sferik refraktif kayma izlenmiştir. Acrysof GİL ile bu kayma en düşük düzeyde olmakla beraber PMMA GİL hariç tüm GİL tipleri ameliyat öncesi sferik refraksiyon ile kıyaslandığında küçük ve kabul edilebilir miyopik kayma göstermiştir.
