Bilateral iridoschisis and unilateral angle closure glaucoma

Abstract
Iridoschisis is a rare degenerative condition of the iris stro- ma that occurs bilaterally in elderly patients. It is usually associated with angle-closure glaucoma. A-45 year-old woman presented to the ophthalmology de- partment with change in the color of the left eye. Her best- corrected visual acuity was 45/50 in both eyes. Intraocular pressures with applanation were 16 mmHg OD and 25 mmHg OS, respectively. On slit-lamp examination, more in the left eye, atrophy of the iris and fibrillary degeneration of the iris stroma were seen bilaterally. Fundus examina- tions of both eyes were normal. On gonioscopy, grade 2 angle in both eyes was determined with peripheral ante- rior synechiae. Nd-YAG laser iridotomy and topical anti- glaucomatous therapy were performed on the left eye. Angle-closure glaucoma with forming peripheral anterior synechiae occuss in about 90% of iridoschisis cases, but this condition could be asymmetric the affected eye must be treated and the other eye must be followed in this con- dition.

İridoskizis ileri yaşlarda iki taraflı olarak ortaya çıkan iris stromasının dejeneratif bir hastalığıdır. Çoğunlukla açı ka- panması glokomu ile birlikte görülür. Kırk beş yaşındaki bayan sol göz renginde değişiklik şika- yeti ile hastanemize başvurdu. Düzeltilmiş görme keskinliği her iki gözde 0.9 düzeyindeydi. Göz içi basıncı aplanasyon ile sağ gözde 16 mmHg, sol gözde ise 25 mmHg olarak ölçüldü. Biyomikroskopik muayenede sol gözde daha fazla olmak üzere her iki gözde iriste atrofi ve iris stromasındaki liflerde dejenerasyon görüldü. Fundus muayenesinde her iki gözde normal muayene bulgularına rastlandı. Gonyos- kopide periferik ön sineşilerle birlikte her iki gözde ikinci derecede açı varlığı tespit edildi. Hastanın sol gözüne Nd- YAG lazer iridotomi uygulandı ve topikal antiglokomatöz tedavi başlandı. İridoskizisli vakalarda %90 oranında periferik ön sineşi oluşumu ile glokom ortaya çıkmaktadır. Fakat bu durum asimetrik olabilmektedir. Böyle bir durumda etkilenen göz tedavi edilmeli ve diğer göz dikkatle takip edilmelidir.
