Importance of dynamic contour tonometry to evaluate the ocular pulse amplitude

Abstract
Purpose: The purpose of this investigation is to evaluate intraocular pressure (IOP) and ocular pulse amplitude (OPA) as parameters of ocular circulation balance,which were measured by dynamic contour tonometry (DCT) (Pascal;Swiss Microtechnology AG, Zurich,Switzerland) and pulsatil ocular blood flow analyser of Paradigm (Dicon Ocular Bşlood Flow Analyser) . Materials and Methods: Thirty four eyes of 17 healhty subjects were included the study. Nine of the patients were women and 8 of them were with age range of 50 to 75 years. IOP and OPA were measured under topical anesthesia and in sitting posture by DCT with Q= 1-3 taken in to account. After ten minutes, the same values were measured by POBF analyser. The results were evaluated. Results: On 34 healthy eyes; 1. Intraocular pressure values were 16.0±2.4 mmHg by DCT and 15.9±3.3 mmHg by POBF. There was a strong correlation between DCT and POBF (r=0.504, p<0.002). The difference between the mean IOP was not statistically important (t test; t0. 15, p>0.05).There was relation with age. 2. Ocular pulse amplitude values were 2.8±0.8 mmHg by DCT and 4.0±1.3 mmHg by POBF. There was a correlelation between the results of OPA values from DCT and POBF (r=0.643, p<0.001, but the values from POBF were significantly higher than those of DCT. There was no relation with age. Conclusion: POBF analyser’s IOP values was the same as DCT’s IOP values; but POBF-OPA was higher than DCTOPA and could be correlated with each other. Since corneal parameters do not effect to DCT measurements, simultaneous IOP and OPA measurements with this tonometer (DCT) is needed for measurement of the parameters of ocular circulation balance in glaucoma clinics and target OPA may be a good criteria for treatment choice.

Amaç: Bu çalışmanın amacı oküler dolanım denge terazisi parametreleri olan göz içi basınçı (GİB) ve oküler nabız amplitüdü (ONA) nü eşzamanlı ölçme yeteneğinde olan dinamik kontur tonometre (DKT/Ziemer) ve POBF analizörü (Paradigm) sonuçlarının klinik pratiğinde değerlendirmektir. Gereç ve Yöntem: Yaşları 50 ile 75 arasında olan 9’u erkek 8’i kadın, toplam 17 olgunun 34 sağlıklı gözü çalışmaya dahil edilmiştir. Topikal anestezi altında ve oturur konumda önce sağ ve sonra sol gözde DKT ile 1, 2, 3 kalitesinde GİB ve OPA ölçümü yapıldıktan 10 dakika sonra, POBF analizörü ile aynı parametrelere (GİB ve OPA) ölçümleri yapılmıştır. Bulgular: Yaşları 50-75 arasındaki sağlıklı olguların 34 gözünde; GİB ölçümleri DKT ile ortalama: 16.0+/2.4 mmHg, POKA-GİB ortalama: 15.9+/-3.3 mmHg bulunmuştur. Aralarındaki fark anlamsız (SD: 68, t: 0.15 p>0.05) bulunmamaktadır. DKT-OPA ortalama: 2.8+/-0.8 mmHg, Paradigm- OPA ortalama: 4.0+/-1.3 mmHg aralarındaki fark anlamlı (SD: 68, t: -4.7 p<0.05) bulunmuştur. Yaşla ilişkileri vardır. Sonuç: GİB ölçümünde heriki yöntem uyumlu olmasına karşın ONA değerlendirmesi arasında güçlü korelasyon saptanmıştır. DKT klinik pratiğinde daha geçerlidir.
