Peroperative İntracameral triamcinolone acetonide İnjection in a pediatric case of uveitic cataract

Abstract
A 4-year-old boy was diagnosed with bilateral cataracts and uveitis. Bilateral cataract surgery was planned with the right eye being operated on first. During the surgery on the right eye an intravenous steroid injection of 250 mg was applied and after the surgery an intracameral injection of triamcinolone acetonide (0.04 mg/0.1 ml) was applied. During the operation on the left eye the same dosage intravenous steroid injection was employed but an intracameral steroid injection was not used. The postoperative treatment protocol was the same for the two eyes. In the right eye, during the 12 months following, a fibrin reaction did not occur , but in the left eye a fibrin reaction occurred after the third postoperative day. This reaction persisted for two months and then the left eye was re-operated on and the fibrin was removed. In the right eye, during the follow up period, there was no increase in intraocular pressure and there were no inflammatory reactions either. It is known that after pediatric cataract surgery the inflammatory reaction is more severe than that after adult cataract surgery, and conversely the compliance of children to the treatment is low. Our patient had uveitis and a very high risk for an inflammatory reaction. In such patients, after cataract surgery, to prevent a postoperative inflammatory reaction, we can use an intracameral triamcinolone injection (0.04 mg/0.1 ml) confidently.
Özet
Dört yaşında erkek hastaya, her iki gözünde üveitik katarakt tespit edilerek, sağ öncelikli bilateral katarakt ameliyatı planlandı. Sağ gözün ameliyatı sırasında intravenöz (iv) 250 mg steroid injeksiyonu ve ameliyat bitiminde intrakameral 0.04 mg/0.1 ml triamsinolon asetonid (TA) injeksiyonu uygulandı. Sol gözün katarakt ameliyatı sırasında ise sadece ameliyat sırasında 250 mg intravenöz steroid injeksiyonu uygulandı ve intrakameral TA injeksiyonu yapılmadı. Her iki göze aynı ameliyat sonrası tedavi protokolü uygulandı. Hastanın sağ gözünde ameliyat sonrası 12 ay süresince fibrin reaksiyonu gözlenmezken sol gözünde ameliyat sonrası 3. gün gelişen fibrin reaksiyonu 2 ay boyunca takip edildi ve gerilememesi nedeniyle tekrar ameliyatla fibrin temizliği uygulandı. Sağ göz takibi boyunca göz içi basıncında antiglokomatöz tedavi gerektirecek artış olmadı. Bilindiği gibi pediatrik olgularda katarakt ameliyatı sonrası inflamatuar reaksiyon daha şiddetli olmakla birlikte hastanın ameliyat sonrası tedaviye uyumu da oldukça zordur. Bizim olgumuz aynı zamanda üveitik katarakt olgusuydu ve fibrin reaksiyonu geliştirme potansiyeli daha yüksekti. Böylesi olgularda katarakt cerrahisi bitiminde intrakameral 0.04 mg/0.1ml TA injeksiyonunun ameliyat sonrası inflamasyon oluşumunu uzun süre engelleyeceği ve oldukça güvenli olarak kullanılabileceği kanaatindeyiz.
