New Therapeutic Approaches in Glaucoma: What's in the Future?

Abstract

Glaucoma, as a multifactorial, progressive and neurodegenerative group of diseases, ends up visual loss without appropi-ate treatment. There are cases with optic nerve damage and progression even in the presence of the normal intraocular pressure (IOP). In that case another mechanisms play role. It is possible to stop progression and control ganglion cell loss through understanding these mechanisms. Then effective treatment strategies might be developed. In this article, damage mechanisms shedding light on glaucoma physiopathology, related current treatment methods and the ideas and studies answering the question of what could be done in the future will be discussed.

Glokom, multifaktöryel, progresif ve nörodejeneratif bir hastalık grubu olup, tedavi edilmediğinde görme kaybı ile sonuçla-nır. Göz içi basınç (GİB) yüksekliği halen kontrol edilebilir tek majör risk faktörü olmakla birlikte, GİB kontrolüne rağmen ilerleme gösteren ya da GİB normal seviyede olmasına rağmen optik sinir hasarı gelişen olgular söz konusudur. Bu durum-da yüksek GİB'ndan bağımsız başka mekanizmalar devreye girmektedir. Glokomda ganglion hücre hasarının kontrol altına alınması ve ilerlemenin durdurulması bu mekanizmalara ait fizyopatolojinin iyi anlaşılması ile mümkündür. Sonuçta etkin tedavi yöntemleri ancak bu şekilde geliştirilebilir.Makalede glokomun fizyopatolojisine ışık tutan hasar mekanizmaları, bunlara ait güncel tedavi yaklaşımları ve gelecekte neler yapılabilir sorusunu aydınlatmaya yarayacak çalışma ve fikirler etraflıca irdelenecektir.
