A case of familial adenomatous polyposis with bulber polyposis

Abstract

Familial adenomatous polyposis (FAP) is a familial disease inherited as an autosomal dominant. It is characterised by the progressive development of hundreds to thousands of adenomatous polyps in the large intestine. If the colon is not removed, the development of colon cancer is virtually inevitable. Patient with FAP are known to be high risk for duodenal cancer. We have to follow these patients because the adenoma-carcinoma sequence is thought to exist in the duodenum, like colon. On the other hand, there is discussion about either medical or surgical treatment of the patient with FAP and duodenal polyposis. In this case, we present a patient with FAP and discuss doudenal polyposis.

Özet

Familiyal adenomatöz polipozis (FAP) otozomal dominant geçişli, kolonda yüzlerce adenomatöz polip gelişimi ile karakterize bir hastalıktır. Kolektomi yapılmazsa kolon kanserinin gelişimi kaçınılmazdır. FAP'lı hastalarda duodenumda da polipler görülebilmekte ve bu polipler de kanser riski taşımaktadır. FAP'da kolonda gözlenen adenom-karsinom dönüşümünün duodenumda da oluşması bu hastaların sıkı takibinin önemini arttırmaktadır. Ayrıca duodenum polipi olan FAP'lı olguların gerek medikal, gerekse cerrahi tedavisinin tartışmalı olması da önemli bir problemdir. Bu yazıda, duodenal polipozis saptanan bir FAP olgusu sunulmaktadır.
