PUVA-alone, bath-PUVA and re-PUVA in the treatment of psoriasis: A clinical comparison

Abstract
Psoriasis is a genetic disease that needs to be treated with therapeutic modalities which must be less unpleasant and less intolerable than the psoriasis itself. PUVA, either alone or combined with other treatments, is still a good alternative to toxic therapies in the treatment of psoriasis. Fifty patients with psoriasis were enrolled into the study and were allocated into three groups. Group A (n=20) were treated with acitretin in combination with PUVA (Re-PUVA), Group B (n=20) were treated with bath-PUVA (trimethylpsoralen bath plus PUVA) and Group C (n=10) were treated with PUVA alone. Patients were evaluated with Psoriasis Area and Severity Index (PASI) score before and after treatments. Side effects were also monitored. In Group A; of 20 patients, six were cleared completely, 14 were cleared partially. In Group B; 19 out of 20 patients cleared completely and one showed partial response. In Group C, of 10 patients, six were cleared completely, three were cleared partially and one showed poor response. All side-effects were mild and well-tolerated. In conclusion, although photochemotherapy of psoriasis should be individually tailored, one should prefer bath- or re-PUVA therapy as first line treatment, because they need less UVA to clear psoriasis than PUVA alone as shown in our study.

Psoriasis, kendi oluşturduğu rahatsızlıklardan daha kolay tolere edilebilen ve daha az rahatsızlık verici yöntemlerle tedavi edilmesi gereken genetik bir hastalıktır. PUVA, gerek tek başına, gerekse başka tedavilerle kombine edildiğinde, toksik tedavilere hala iyi bir alternatiftir. Çalışmaya 50 psoriasisli hasta alındı ve bunlar 3 gruba ayrıldı. Grup A (n=20) asitretin ve PUVA kombinasyonu (Re-PUVA) ile, Grup B (n=20) banyo-PUVA (trimethylpsoralen banyosunu takiben PUVA) ile ve Grup C ise (n=10) tek başına PUVA ile tedavi edildi. Hastalar tedavi öncesi ve sonrasında psoriasis alan ve şiddet indeksi (PASI) ile skorlandılar. Yan etkiler kaydedildi. Grup A' da; 20 hastanın 6'sı tamamen, 14'ü ise kısmen, Grup B' de; 20 hastanın 19'u tamamen ve biri kısmen, Grup C' de ise 10 hastanın 6'sı tamamen üçü kısmen düzelirken, birinde yanıt çok azdı. Bütün gruplarda rastlanan yan etkiler hafif ve tolere edilebilir düzeydeydi. Sonuç olarak, psoriasisin fotokemoterapötik tedavisi hastaya özgü planlanmalıdır. Fakat banyo-PUVA ve Re- PUVA tedavilerinde, bizim çalışmalarımızda da gösterdiğimiz gibi, psoraisisin düzelmesi için gereken toplam UVA dozunun daha düşük olduğu göz önüne alındığında, kombine tedavilerin tek başına PUVA' ya tercih edilebileceğini düşünüyoruz.
