Mechanisms of urinary erythrocyte dysmorphism

Abstract
Source of hematuria could be estimated according to morphology of urinary erythrocytes. Glomerular erythrocytes are called as 'dysmorphic' and non-glomerular erythrocytes as 'isomorphic'. The pathogenetic mechanism for urinary erythrocyte dysmorphism remains undefined. Glomerulonephritis cause disruption of the glomerular and peritubular capillaries, thus, allowing erythrocytes to escape into the renal tubuler lumen. As erythrocytes are forced through abnormal capillaries the cell walls are damaged giving them a distorted, dysmorphic shape. Other possible explanations for dysmorphism are physical disruption of the erythrocytes during passage through gaps in the glomerular basement membrane, the presence of toxic enzymes resulting from inflammatory processes, phagocytic activity of tubular epithelial cells, exposure to the dynamic changes of the osmotic or enzymatic environment when filtered erythrocytes pass through the tubuler system; finally, a combination of these mechanisms.

Hematürinin kaynağı, idrarda bulunan eritrositlerin morfolojik görünümlerine göre tahmin edilebilir. Glomerüler eritrositler 'dismorfik', nonglomerüler eritrositler 'izomorfik' olarak adlandırılmaktadır. Üriner eritrositlerde meydana gelen dismorfizmin patogenezi belirsizliğini korumaktadır. Glomerülonefritler, glomerüler kapillerlerde parçalanmaya neden olarak, eritrositlerin renal tübüler lümen içine atılmasına neden olurlar. Anormal kapillerlerin içinden geçerken sıkışan eritrositlerin membranı, bozularak hasara uğrar ve dismorfik şekiller oluşabilir. Dismorfizm için diğer olası mekanizmalar; glomerüler bazal membrandaki aralıklardan geçişleri sırasında eritrositlerin uğradığı fiziksel stress, inflamatuar proçesten kaynaklanan toksik enzimlerin bulunması, tübüler epitel hücrelerinin fagositik aktivitesi, tübüler sisteme geçen eritrositlerin maruz kaldıkları dinamik ozmotik ve enzimatik değişiklikler ve sonuç olarak bu mekanizmaların bir kombinasyonu.
