Results of interferon treatment in chronic hepatitis B

Abstract
Data related to long-term results of interferon (IFN) therapy in chronic hepatitis B is not sufficient at pre¬sent although there are lots of studies and results including short-term outcomes. In this study; results of IFN treatment obtained in the short term and at the end of different follow-up periods in patients with chronic hepatitis B in our clinics were investigated retrospectively. Overall 50 patients (28 male, mean age 32) diag¬nosed chronic hepatitis B and given IFN treatment were evaluated with their serologic, biochemical and histopathological findings. 25 patients were treated with IFN alpha-2a and 25 with alpha-2b (there times weekly, 5 or 9 million units) for six months. At the end of the treatment; complete response (loss of HBV-DNA in serum, normalization of ALT levels) was obtained in 29 (58 %) patients and partial response (persistence of HBV-DNA in serum, normalization of ALT levels) in 2 (4%) patients. There was no response in the remaining 19 (38%) patients. All patients were followed-up 12-42 months (mean 24 months). During the follow-up, relaps (reproduction of HBV-DNA in serum and rising in ALT levels) occurred in 13 patients. Clinical and laboratory findings of the other 18 patients (36% of all cases) were normal in the course of the follow-up. In the histopathologic evaluation of the cases with complete response, it was found that hepatocelluler inflammation evidently improved at the end of the treatment. Cirrhosis or hepatocellular carcinoma were not seen in any of these cases either while 2 unresponders died because of acute liver failure. HBs Ag didn't become negative in any case. Response rate to IFN in the treatment of chronic hepatitis B isnt in a sufficient level. However, long-term remission in association with HBV-DNA negativ¬ity can be obtained in the vast majority of the patients with response.

Kronik hepatit B'de interferon tedavisinin erken sonuçlarına ait çok sayıda çalışma ve veri bulun¬masına rağmen, uzun dönem sonuçları ile ilgili bil¬giler halen yeterli değildir. Bu çalışmada; kliniğimizde İFN tedavisi uygulanan kronik B hepatitli hastalarda erken dönemde ve değişik takip süreleri ile elde edilen sonuçlar retrospektif olarak araştırıldı. Kronik hepatit B tanısı konulan ve interferon tedavisi uygulanan toplam 50 hasta (28 erkek, 22 kadın; yaş ortancası 32); serolojik, biyokimyasal ve histopatolo-jik bulguları ile değerlendirildi. Altı ay süreyle, 25 has¬taya interferon alfa-2a ve 25 hastaya interferon alfa-2b uygulandı (haftada üç defa, 5 veya 9 milyon ünite). Tedavi sonunda; 29 (%58) hastada tam yanıt (serumda HBV-DNA'nın kaybolması, ALT düzey¬lerinin normale dönmesi), 2 (%4) hastada parsiyel yanıt (serum HBV-DNA pozitifliğinin devam etmesi, ALT düzeylerinin normale dönmesi) alındı. Geriye kalan 19 (%38) hastada tedaviye yanıt alınamadı. Tüm hastalar 12-42 ay (ortalama 24 ay) süreyle takip edildi. Takip sırasında; 13 hastada relaps (serumda HBV-DNA'nın yeniden saptanması ve anormal serum aminotransferaz düzeyleri) meydana geldi. Diğer 18 hastada (tüm olguların %36'sı) klinik ve laboratuvar bulguları takip süresi boyunca normaldi. Tedaviye tam yanıt alınan olguların tedavi sonu histopatolojik değerlendirilmelerinde, hepatosellüler inflamasy-onun belirgin şekilde gerilediği tespit edildi. Yine bu olgularda takip süresi boyunca siroz veya hepatosel¬lüler karsinoma tesbit edilmemesine karşılık, tedaviye cevapsız iki hasta akut karaciğer yetmezliği nedeniyle kaybedildi. Hiçbir olguda HBs Ag negati-fleşmedi. Kronik B hepatiti tedavisinde interferona yanıt yeterli oranda değildir. Ancak tedaviye cevap veren hasta¬ların çoğunda HBV-DNA negatifliği ile giden uzun süreli remisyon sağlanabilmektedir.
